a. BACKGROUND:
With funding from the Global Fund, Family Health International (FHI 360) is collaborating with Achieving Health Nigeria Initiative (AHNi) to support the government of Nigeria to enhance sustained cross-sectional integration of HIV/AIDS services by building Nigeria’s capacity to deliver sustainable high quality, comprehensive, prevention, treatment care and related services in Nigeria.
Achieving Health Nigeria Initiative (AHNI) I inviting qualified and interested companies to submit sealed quotations for the supply of Medical Laboratory Consumables
b. REQUEST:

c.MINIMUM ELIGIBILITY REQUIREMENT AND AWARD CRITERIA:
No Tender Requirement 1 Company Profile; registered address(s), official/functional emails, telephone numbers and point of contact for the company: 2 Evidence of legal registration document of the company (CAC, FORM CO7 and FORM CO2) 3 Tax Clearance Certificate for 20172018 and 2019; 4 Evidence of Financial Capability (most recent audited account): 5 Bank Reference Letter duly addressed to AHNI Procurement Committee: 6 Evidence of experience in award(s) of similar scope and complexity within the last 3 years: 7 Bidders must provide copies) of Job Orders and proof of delivery(ies); 8 Financial Quotation and required Commercial Terms.
d. EVALUATION AND SELECTION CRITERIA:
Bidders must ensure that all requested information above are included in the submission will be conformity to the tender requirement listed. Specification, Quality, Delivery Lead Time Price Competition, Payment Terms and Warranty Submission Proposals will be evaluated based on completeness and Provision)
To ensure AHNi get value for money. Bidders that meet the technical requiem will be required to submit physical samples of the product proposed for physical verification approval and shall form part of the selection process.
e. SUBMISSION PROCEDURE AND INSTRUCTIONS:
Technical Documentation (Criteria and Financial Quotation including commercial terms (Criteria 7) MUST be separated signed by the authorized representative and in send envelopes. Your submission must be marked with the Tender Reference Number. Completed tender documents should be addressed and submitted in hard copy to:

AHNi is Value Added Tax (VAT) exempt but deducts withholding tax (WHT)in compliance with relevant Nigeria Tax Law,
Deliveries these consumables are to be delivered to Central Medical Store, Lagos, Nigeria:
All proposal document submitted including prices must be valid for six (6) month from the Tender due date. This includes but not limited to cost, terms and conditions and master level agreement.
g. CLOSING DATE:
Tender document must be hand delivered or couriered to the above address on or before Thursday 30 January 2020 at 5:00 pm prompt.
