INVITATION FOR PREQUALIFICATION
1. This invitation for prequalification follows the Oyo State Ministry of Health proposed Drug Distribution Centre to conform to the National policy of providing drugs directly from drug manufacturers in selected hospitals and Public Private Partnership (PPP) in other hospitals.
2. Prequalification will be conducted through prequalification procedures specified in the Bureau of Public Procurement Guidelines, and is open to all bidders from the country as defined in the guidelines.
3. Interested eligible applicants may obtain further information from and inspect the prequalification document at the Department of Planning, Research and Statistics, Oyo State Ministry of Health, Secretariat Ibadan.
4. Interested bidders who are strictly corporate bodies registered with Corporate Affairs Commission must have the following:
i. Current Registration with the Bureau of Public Procurement Oyo State.
ii. Current (2020) Certificate of Registration with Pharmacist Council of Nigeria
iii. Current (2020) Annual License to practice of the superintendent pharmacist From Pharmacist Council of Nigeria.
iv. Current Business Premises Registration with Oyo State Ministry of Commerce and Cooperative.
v. Current (2020) Tax Clearance Certificate for three years issued by the Board of Internal revenue, Oyo State.
vi. Current (2020) personal Income Tax deduction of two directors of the company.
vii. Evidence of similar jobs done.
viii. Company profile.
ix. Government Development Levy
x. Any other relevant information
5. A complete set of the prequalification document in English language may be purchased by interested applicants upon payment of a non-refundable fee of One Hundred Thousand Naira (N100, 000.00) in bank draft payable to the Oyo State Government.
6. Application for prequalification should be submitted in sealed envelopes, delivered to the address below and be clearly marked “Application to prequalify for Drug Distribution Centre/PPP”
7. Bids must be delivered to the address below on or before 19th March, 2020. Late bid will be rejected. Bids will be opened in the presence of the bidders designated representatives and anyone who chooses to attend at the address below on Thursday 19th March 2020 by 12 noon.
