REQUEST FOR QUOTATION (RFQ) PROVISION OF WATER SANITATION AND HYGIENE (WASH) SERVICES (DRILLING OF WATER BOREHOLE AND CONSTRUCTION OF LATRINES AND SHOWER STALL)
A. BACKGROUND:
FAMILY HEALTH INTERNATIONAL (FHI360) is a not-for-profit human development organization dedicated to improving lives in lasting ways by advancing integrated, locally driven solutions. Our staff includes expert in health, education, nutrition, environmental, economic development, civil society, gender equality, youth, research, and technology, creating a unique mix of capabilities to address today’s interrelated development challenges. With funding from USAID’s Office of Foreign Disaster Assistance (OFDA) and SNEPCO, FHI 360 is supporting acute needs of internally displaced persons (IDPs) in Borno State through the provision of integrated health and nutrition, protection and WASH programming.

C. EVALUATION FACTORS & SELECTION CRITERIA:
Bidder should ensure that requested information are included in the submission. Proposals will be evaluated based on the following: conformity to RFQ documentation requirement, conformity to the Bill of Quantities and specifications, competitive prices, validity of quote, warranty terms and delivery terms. Bidders can bid for a single or multiple lot.
D. DOCUMENTATION REQUIRED:
Please review the table below for required components of the proposal submissions. Evaluators will neither make assumptions nor draw inferences regarding missing or incomplete information. The submission of incomplete information could result in the rejection of the proposal.
No Document required 1 Company profile with registered address, functional email address and telephone number, point of contact for the entity. 2 Evidence of company registration (.e. Certificate of incorporation, form (CAC7 & CAC2). 3 Evidence of tax payment for the last three years Provide verifiable evidence of past performance of similar projects, that is, signed 4 Contract/Purchase Order and evidence of completion like Job Completion Certificate for INGO, Government institution and reputable private organization 5 Three professional references, including name, phone number and organization that can attest to satisfactory performance in previous provision of related supply service. 6 Quoted price(s) for each of the Lot you are responding to.
E. INSTRUCTION ON SUBMISSION:
I. Your biding document/proposal must include the required documents listed above documents and other relevant document should be scanned in PDF format and submit electronically to WASH.Services@fhi360.org latest 5pm 29 April 2020 (Physical submission is not required).
II. Each document must be in PDF Format, clearly titled and submit as an attachment using your your email account.
III. The subject of the email must be quoted as “Proposal for Provision of Water Sanitation and Hygiene (WASH) Services” by (insert your company name here).
IV. Quotation for each Lot must be on a separate PDF document/format of your letterhead and signed by the authorized person(s).
V. Multiple submission of same document/proposal is not allowed. Should you submit proposal more than once, you MUST send comment and reference the proposal/email to ignore.
F. BID CLARIFICATION:
Clarification or enquiry regarding this RFQ should be sent via email to WASH.Services@fhi360.org by 5pm April 23″, 2020.
PLEASE NOTE:
Any proposal received by FHI360 after the deadline for submission stated herein shall be disqualified FHI360 anticipates awarding multiple contracts but reserves the right to terminate the RFQ or negotiations at any time prior to award.
Issuance of this RFQ in no way constitutes a commitment by FHI360 to award a contract. FHI 360 reserves the right to vary any part of this RFP at the time of prior to award if it becomes necessary.
This procurement is funded is funded by the US Agency for International Development (USAID) and is subject to the applicable rules and regulations of US Government Funded awards.
FHI360 is Value added tax (VAT) exempted, but charges Withholding Tax (WTH) in compliance with Relevant Nigerian laws.
