CONTROL/PERSONAL PROTECTION EQUIPMENT

A. Background:
Family Health International (FHI360) is not-for-profit human development organization dedicated to improving lives in lasting ways by advancing integrated, locally driven solutions. FHI360 is implementing the USAID’s Office of Foreign Disaster Assistance (OFDA), Integrated
Humanitarian Assistance Northeast Nigeria IHAAN I1). FHI360 is supporting the acute needs of internally displaced persons (1DPs) in Borno State through the provision of integrated health and nutrition, protection and WASH programming. FHI360 has received funds from USAID to intervene in Banki, Dikwa and Ngala local governments to mitigate the spread of the COVID 19 virus and reduce the risk of infection in an already vulnerable displaced population.
FHI360 is hereby seeking qualified and interested vendors to submit quotations for the supply of the following Infection, Protection & Control.consumables to our Maiduguri Office.

C. Evaluation Factors & Selection Criteria:
Bidders should ensure that requested information is included in the submission. Proposals will be evaluated based on the following: conformity to RFQ documentation requirement, conformity to specifications, competitive prices, validity of quote, warranty terms and delivery terms.
D. Documentation Required:
Please review the table below for required components of the proposal submissions.
Evaluators will either make assumptions nor draw inferences regarding missing or incomplete information. The submission of incomplete information could result in the rejection of the proposal.
No. Document required 1 Company profile with registered address, functional email address and telephone number, point of contact for the entity. 2 Evidence of company registration (i.e. Certificate of incorporation, form CAC7 & CAC2). 3 Evidence of tax payment for the last three years (that is, 2017,2018 & 2019 TCC) 4 Evidence of similar projects for NGOs, Government and Private Institutions/organizations). The evidence should include Purchase Orders, Award Letters and Good Received Note/Delivery Note indicating successful supply and signed by the authorized person(s) from the contracting entity (ies) 5 Three professional references, including name, phone number and organization that can attest to satisfactory performance in previous provision of related supply service. 6 Product catalogue (including specification and pictures of the product to be supplied) 7 Quotation price (s) for the required items. Your quotations should state the Source and Original of the items they intend to supply if awarded. Please note that Vendors, if awarded, will be required to confirm that none of the PPE’S intended for the supply are meant for the U.S, market.
E. Instruction on submission:
Complete proposal must include the required documents listed above and the cost documents should be on your company letterhead signed by the authorized person(s) and submitted to the underlisted address latest 5 pm August 27th, 2020.

F. Bid Clarification:
Clarification or enquiry regarding_this RFQ should be sent via
quotations.Nigeria@ng.fhi360.org on or before 5 pm August 18th, 2020. FHI360 will provide a response to all queries regarding this RFP by the same date.
Please note:
Any proposal received by FHI360 after the deadline for submission stated herein shall be disqualified.
FHI360 anticipates awarding multiple contracts but reserves the right to terminate the RFQ or negotiations at any time prior to award.
Issuance of this RFQ in no way constitutes a commitment by FHI360 to award a contract. FHI360 reserves the right to ary any part of this RFP at the time of prior to award if it becomes necessary
This procurement is funded by the U.S Agency for International Development (USAID) and is subject to the applicable rules and regulations of US Government Funded awards.
FHI360 is Value added to (VAT) exempted, but charges Withholding Tax (WTH) in compliance with Relevant Nigerian laws.
