1. Background
Heartland Alliance Ltd/Gte (HALG) is an indigenous non-governmental organization established to address the human rights needs of vulnerable persons in the society. HALG currently operates in 3 states (Lagos, Akwa-Ibom and Cross Rivers) with over 190 employees working at different project sites across the states.
Heartland Alliance LTD/GTE has the need to lead line their mobile Digital X-ray rooms in seven locations:

2. Major Objectives /Deliverables:
To lead line mobile Digital X-ray rooms in seven locations:. Akwa Ibom (2 Locations), Cross
Rivers (2 Locations) and Lagos (3 Locations).
(i) (3 Sided Lead Shield)
(ii) Supply 2 lead rubber aprons for each location
(iii) Supply 2 Anatomic marker (R/L and L/R for each location
(iv) 1 Stool in each location
(v) Air Conditioner (2 nos) in each location
(vi) Bed side Screen or Changing cubicle in each location
(vi) X-ray warning light at the entrance in each location
(vii) Office desk and chairs for the radiographer’s room in each Location
(ix) Safety Goggles in each location
(x) Safety Rubber gloves in each location
(xi) Safety Rubber boots in each location
(xii) 1 13amp or 15amp socket depending on the machine in each location.
3. Expected duration of Assignment
The task is expected to be completed on or before 2 weeks from when Agreement is signed.
The consultant, therefore, is required to prepare and submit the work plan in a way that allows the achievement of all deliverables within this set time
4. Submission Requirements
Please include the following in your submission:
-Project proposal with work plan outline and approximate time frame (maximum 5 pages)
-A financial quotation must be in Naira
-Evidence of past performance in similar assignment
-Company’s profile with verifiable address, telephone numbers, email addresses, and organization’s administrative structure where applicable
-Evidence of Corporate Affairs Commission registration (CAC), i.e. Company certificate of Incorporation.
-Evidence of TIN, and photocopy of Tax clearance Certificate
5. Method of Response
Interested firm or Consultant should submit a proposal quoting
