INVITATION FOR PREQUALIFICATION AND TENDER IN RESPECT OF PHASE II CONSTRUCTION WORKS AT SPECIALIST HOSPITAL HADEJIA, SPECIALIST HOSPITAL KAZAURE, SCHOOL OF NURSING MIDWIFERY BABURA AND NEW GENERAL HOSPITAL GURI AND CONSTRUCTION OF 3 NEW GENERAL HOSPITALS AT GAGARAWA, GWIWA AND KIRIKASAMMA
PROCUREMENT NO MOH/STB/01 -09/2021
In Compliance with Jigawa State Procurement Process, Jigawa State Ministry of Health wishes to notify the interested bidders of its intention to carry out Phase II Construction Works at School of Nursing and Midwifery Babura, Specialist Hospital Kazaure, Specialist Hospital Hadejia, and New General Hospital Guri, and Construction of 3 New General Hospitals at Gagarawa, Gwiwa and Kirikasamma.
(2) PROJECT SCOPE:
The Projects to be earned out include:
Lot 1: Construction of Additional Student’s Hostel (1 Block) and External Mechanical and Electrical Installation at School of Nursing and Midwifery Babura
Lot 2: Construction of External Road Network and Drainage at School of Nursing and Midwifery Babura
Lot 3: Phase II Construction works at Specialist Hospital Kazaure (Extension of Road Network, Drainage, Construction of Staff Quarters and Mechanical Installation for all the blocks)
Lot 4: Construction of External Road Network and Drainage at Specialist Hospital Hadejia
Lot 5: Construction of Staff Quarters and Mechanical Installation for all the blocks at Specialist Hospital Hadejia
Lot 6: Construction of New General Hospital at Gagarawa
Lot 8: Construction of New General Hospital at Gwiwa
Lot 9: Construction External Road Network, Drainage and additional structures at New General Hospital Guri.
(3) ELIGIBILITY REQUIREMENTS
Bidding document should be submitted along with following documents:
(i) Certificate of Incorporation with Corporate Affairs Commission
(ii) Certified True Copy of Memorandum and Article of Association
(iii) Evidence of registration with Jigawa State Works Registration Board
(iv) Tax clearance certificate for the last three (3) years
(v) Audited Account for the Last One (1) Year
(vi) VAT Registration and Evidence of Past Remittance
(vii) Evidence of financial capability and banking support
(viii) Company Average Annual Turn Over of not less than N500 Million
(ix) That the Company have fulfilled all obligations to pay taxes, pensions, ITF Contribution and social security contributions
(x) Evidence of execution and completion of similar jobs at least 3 in the last 5 years (Letter of Award and Certificate of Practical Completion to be included).
(xi) Verifiable evidence of Company profile and technical qualification of key personnel registered with relevant professional bodies (COREN, CORBON, QSRBN, ARCON etc.
(xii) A sworn affidavit certifying as Follows:
(a) That the document submitted for the pre-qualification exercise are not only genuine but correct
(b) That none of the directors of the Company has been convicted by any court of law
(c) That the Company is not bankrupt.
(d) That none of the officers of Jigawa State/Local Government is a present or former director of the Company.
(xiii) Any other relevant information that will help in determining the Company’s suitability for the work.
Any prospective Bidder that does not possess any of the above documents need not to apply.
(4) COLLECTION OF TENDER DOCUMENT:
Bidding Document Could be obtained from the office of the Director Planning, Research and Statistics, Ministry of Health Dutse at a non-refundable fee of Three Hundred and Fifty Thousand Naira (N350,000.00) only by 9.00am to 5.00pm from Thursday 28 January, 2021. Bid submission will close on Thursday, 11th February, 2021 by 12.00 noon. Bids will be opened on the same day by 12.00 noon. Prompt.
