INVITATION TO TENDER FOR THE EXECUTION OF YEAR 2021 CAPITAL PROJECTS
1.0 INTRODUCTION
The Federal Medical Centre, Lokoja, hereby invites experienced and competent Contractors/Suppliers to submit bids for the implementation of the year 2021 Capital Projects.
2.0 SCOPE OF WORKS/SUPPLY
Lot 1: Completion of Renovation/Extension/Equipping of male Surgical & Wards
Lot 2: Renovation of Intensive Care Unit/Dialysis Building.
Lot 3: Completion of Procurement & Installation of Medical Equipment.
Lot 4: Completion of Construction (Equipping)of Theatre Suite &CT Scan Building
Lot 5: Medical Outreach (Free Antenatal Drugs) in Lokoja/Kogi Federal Constituency, Kogi State.
3.0 ELIGIBILITY REQUIREMENTS
Prospective bidders should submit their paginated Technical bids containing the following documents:
i Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7;
ii. Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years (2018, 2019&2020) valid till 31st December, 2021;
iii. Evidence of Pension Clearance Certificate valid till 31 December, 2021 (applicable where number of staff is at least 15);
iv. Evidence of Industrial Training Fund (ITF} Compliance Certificate valid till 31st December, 2021 (applicable where number of staff is at least 5 or at least N50m turnover);
v. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2021;
vi. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2021 or valid Certificate issued by BPP.
vii.Sworn Affidavit disclosing whether or not any officer of the relevant committees of Federal Medical Centre, Lokoja or the Bureau of Public Procurement is a former or present Director shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
viii. LOT 5: Evidence of Registration with the Medical & Dental Council of Nigeria, or other related regulatory professional body(ies);ix. Lot 1&2:Evidence of Plants/Equipment with proof of Ownership/Lease;
x. Company’s Audited Accounts for the last three (3) years-2018, 2019 & 2020;
xi. Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
xii. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications;
xiii. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Job Completion Certificates and Photographs of the projects;
xiv. Lots 2,3 & 4: Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria;
xv. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR& Sworn Affidavit are compulsory for each JV partner);
xvi. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letterhead Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
4.0 SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit bid for each of the LOT desired, two (2) spiral bounded hard copies each of Technical bids packaged separately in sealed envelopes and clearly marked Technical with LoT and project Title, at the Top right corner of the envelope. Thereafter, put the two sealed envelopes together in a bigger sealed envelope with the LOT and Project title on the top right-hand corner and addressed to:

Furthermore, the reverse of each sealed envelope should bear the name/address of the bidder and must be registered by the bidder in the Procurement Unit before it is dropped in the designated Tender Box positioned at the entrance to the Board room located in the Central Administrative block Federal Medical Centre, Lokoja, within official working Days/Hours (8am 4pm), not later than: 12:00 noon on Monday, 9 August, 2021.
Note that the Federal Medical Centre, Lokoja, will not be held liable for misplaced or wrongly submitted bids.
6.0 OPENING OF TECHNICAL BIDS
Only Technical bids shall will be opened immediately after the deadlines for submission at 12:00 noon on Monday, 9 August, 2021, in the BoardRoom Federal Medical Centre, No. 1 Salihu Ibrahim Way, Lokoja, Kogi State. 7.0 GENERAL INFORMATION
i. Bids must be submitted in English Language and signed by an official authorised by the bidder;
ii. Bids submitted after the deadline for submission would be returned unopened;
iii. Bidders should not bid for more than one (1) Lot;
iv. The Federal Medical Centre, Lokoja, will not be responsible for any cost or expenses incurred by any interested party(ies) in connection with any response to this invitation and or the preparation or submission in response to any inquiry;
v. Only pre-qualified bidders will be contacted at a later date for financial bidding;
vi. The Federal Medical Centre, Lokoja,is not bound to pre-qualify any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in according with Section 28 of the Public Procurement Act 2007.
