INVITATION TO TENDER FOR SUPPLY AND INSTALLATION OF MEDICAL EQUIPMENT IN SOME STATE HOSPITALS ALONG WITH CONSTRUCTION, RENOVATION AND REMODELING OF SOME STRUCTURES IN THE STATE MINISTRY OF HEALTH HEADQUARTERS, LOKOJA
Tenders are hereby invited from competent and reputable Contractors for the Supply and Installation of Medical Equipment for some State Hospitals along with Construction, Renovation and Remodeling of some Structures in the State Ministry of Health Headquarters Lokoja as below:
LOT-1:Supply and installation of Radiologicaland imaging Equipment to Prince Abubakar Audu University Teaching Hospital (PAAUTH) Anyigba, General Hospital isanlu, Zonal Hospital ldah, Ultra-Modern General Hospital Badoko and Ultra-Modern General Hospital GeguBeki
LOT- 2: Supply And Installation of Special Medical Equipment to Prince Abubakar Audu University Teaching Hospital (PAAUTH) Anyigba, General Hospital Isanlu, Zonal Hospital Idah, Ultra-Modern General Hospital Badoko and Ultra-Modern General Hospital Gegu- Beki

A: Supply And installation of Essential Medical Equipment to Prince Abubakar Audu University Teaching Hospital (PAAUTH) Anyigba
B: Supply and installation af Essential Medical Equipment to General Hospital Isanlu
C: Supply and installation of Essential Medical Equipment to Zonal Hospital Idah
LOT- 4
A: Supply and installation of Essential Medical Equipment to Ultra-Modern General Hospital Badoko
B: Supply and installation of Essential Medical Equipment to Ultra Modern General Hospital Gegu-Beki
LOT- 5: Construction, Renovation and Remodeling of Some Structures in the State Ministry in Health Lokoja.
LOT- 6: Development of Quality Control Laboratory
LOT- 7: Remodeling of three (3) Health Facilities
REQUIREMENTS:
1. Evidence Of Registration with CAC
2. Tax Clearance certificate for past three years
3. Audited Account for the past three years
4. Evidence of Registration with Kogi State Bureau Of Public Procurement,
5. Evidence Of VAT Registration and Remittance
6 Evidence Of financial capability from a reputable Bark in Nigeria
7 Evidence of similar job done in the last three years
8 Comprehensive profile of the Contractor with list of key Personnel.
9 Evidence of Registration of Business Premises in Kogi State.
10 Evidence of payment of appropriate non-refundable Contract fee.
11 Sworn affidavit that none of the Company Director have been convicted of any criminal offence
COLLECTION OF TENDER DOCUMENTS:
Tender documents are obtainable from the Department of Health Planning, Research and Statistics, Ministry of Health, Lokoja on payment of the non-refundable Contract fee applicable to the Lot into a specified Kogi State Board of internal Revenue Bank Account.
SUBMISSION AND OPENING OF BIDS DOCUMENTS:
The duly completed bid documents should be forwarded to the Chairman Ministerial Tenders’ Board, State Ministry of Health, PMB 1068, Lokoja; Attention: Procurement Officer in a sealed envelope with the Lot clearly written on the top left hand corner of the envelope to reach him not later than Twenty One (21) days from the date of this Publication at which time bids will be opened by 12.00 noon in the presence of bidders or their accredited representatives
NOTE
a, The State Policy permits only Enterprises to handle Contracts between zero to One Hundred Million (0-100 million)
b. The Ministerial Tenders; Board reserves the right to reject any or all bids
c. This invitation should not be construed as a commitment on the part of the Ministerial Tenders Board to enter Into any form of contract with any Contractor or into communication with any Contractor nor shall it entitle any Contractor to make claims from the Ministerial Tenders Board
