REQUEST FOR QUOTATION (RFQ) PROVISION OF WATER SANITATION AND HYGIENE (WASH) SERVICES
(A) BACKGROUND:
FHI360 is not-for-profit human development organization dedicated to improving lives in lasting ways by advancing integrated, locally driven solutions. Our staff includes expert in health, education, nutrition, environmental, economic development, civil society, gender equality, youth, research and technology, creating a unique mix of capabilities to address today’s interrelated development challenges. With funding from USAID’s Bureau for Humanitarian Assistant (BHA) and SNEPCo, FHI360 is supporting the acute needs of internally displaced persons (IDPs) in Borno State through the provision of integrated health and nutrition, protection and Water and Sanitation. FHI360 is hereby seeking qualified and interested company to submit quotation for a one Master Service Agreement to provide the below services at the following project locations in Borno State: Bama LGA, Banki LGA, Damasak LGA, Ngala LGA and Jere LGA.

(C ) EVALUATION FACTORS & SELECTION CRITERIA:
Bidder should ensure that requested information are included in the submission. Proposals will be evaluated based on the following: conformity to RFQ documentation requirement (Section D), conformity to the Bill of Quantities and specifications, competitive prices, validity of quote, warranty terms and delivery terms. Bidders can bid (or a single or multiple lot.
(D) DOCUMENTATION REQUIRED:
Please review the table below for required components of the proposal submissions. Evaluators will neither make assumptions nor draw inferences regarding missing or incomplete information. The submission of incomplete information could result in the rejection of the proposal.
No. Document required 1 Company profile with registered address, functional email address and telephone number, point of contact for the entity. 2 Evidence of company registration (i.e. Certificate of incorporation, form CAC7 & CAC2). 3 Evidence of tax payment for the last three years 2020, 2019 and 2018 4 Provide verifiable evidence of past performance of similar projects, that is, signed contract/Purchase Order and evidence of completion like Job Completion Certificate for INGO, Government institution and reputable private organization 5 Three professional references, including name, phone number and organization that can attest to satisfactory performance in previous provision of related supply service. 6 Quoted price(s) for each of the Lot you are responding to and using the downloaded bill of quantity/material as a guide. Each quoted price/priced bill of quantity MUST be on the bidder’s company letterhead and signed by the authorized representative of the company.
(E) INSTRUCTION ON SUBMISSION:
(I) Your biding documents/proposals must include the required documents listed above (Section D, 1 – 6) and other relevant documents. The documents MUST be scanned in PDF format, titled and arranged accordingly. The scanned PDF documents MUST be submitted electronically using your company’s email to WASHservice2022@fhi360.org by 5pm 28 February 2022 latest. Note: Physical submission is not required.
(II) The subject of your email must be quoted as “Proposal for Provision of WASH Services-Lot Number(s) (for company those submitting quote for more than lot).
(III) Quotation for each Lot must be on a separate PDF format company’s letterhead document and signed by the authorized person(s).
(IV) Multiple submission of same document/proposal is not allowed. Should you submit proposal more than once, you MUST comment and reference the proposal/email to ignore.
(F) INSTRUCTION ON BID CLARIFICATION:
Clarification or enquiry regarding this RFQ should be sent via email to quotations.nigeria@ng.fhi360.org by 5pm February 22, 2022.

Any proposal received by FHI360 after the deadline for submission stated herein shall be disqualified
FHI360 anticipates awarding multiple contracts but reserves the right to terminate the RFQ or negotiations at any time prior to award.
Issuance of this RFQ in no way constitutes a commitment by FHI360 to award a contract. FHI 360 reserves the right to vary any part of this RFQ at the time of prior to award if it becomes necessary.
This procurement is funded is funded by the U S Agency for international Development (USAID) and is subject to the applicable rules and regulations of US Government Funded awards.
