1. An implementing partner of the Plateau Comprehensive and Inclusive Eye Health program with funding from CBM is calling for submission of quotations for Ophthalmic medical equipment as listed below with specifications;

Method of Application/Submission of Bid:
Interested vendors should submit quotations via email on or before the October 2022.
All quotation must be submitted electronically to procurementecwaeyecomp@bhuth.org.ng with the subject; PCOMIEH- Ophthalmic Equipment. Quotations must be signed, stamped, scanned and sent as an attachment in PDF format.
Instructions
- All quotations must be submitted before the close of business (4pm) on the 7th October 2022
- Bidders must follow the instructions set forth in the advert for tender or bids will not be considered.
- A prospective bidder requiring any clarification may verify with us via email no later than 5th October 2022 at the email address ruth.alfin@yahoo.corn.
- Provide scanned evidence of registration of business name and with a verifiable business address and a valid bank account in the name of the business.
- Provide evidence of CAC registration, payment of Tax/Company TIN and Tax clearance certificate for 2021
- We will not be responsible for any cost or expenses incurred by the vendor in connection with the preparation of submission of bid.
- Delivery and installation of equipment will be done at three locations namely: BHUTH Jos, COCIN Hospital Mangu and GH Pankshin.
Evaluation criteria.
Bids will be evaluated based on their responsiveness to the specifications, qualification, after sales services, payment terms and delivery time.
NB:
Applicable 5% withholding tax (WHT) will be deducted.
