INVITATION TO TENDER FOR THE PROVISION OF MEDICAL EQUIPMENT, FURNITURE & FITTINGS FOR THE MODEL PRIMARY HEALTHCARE CENTRE (PHC), BOJIBOJI AGBOR, IKA SOUTH LGA, DELTA STATE AND JUMBERI PRIMARY HEALTHCARE CENTRE (PHC), KATAGUM LGA, BAUCHI STATE UNDER THE ADOPT-A-HEALTHCARE FACILITY PROGRAM (ADHFP)
The Private Sector Health Alliance of Nigeria invites all prospective bidders to apply for eligibility and subsequently bid for the furnishing and provision of basic medical equipment to Model Primary Healthcare Centre (PHC), Boji Boji-Agbor, Ika South LGA, in Delta State and Jumberi Primary Healthcare Centre (PHC), Katagum LGA, Bauchi.
The works include but are not limited to providing basic Primary Health Care Centre (PHC) furnishings, fittings, and small sized medical equipment.
The conditions for the prospective tenderers are listed as follows:
(1) There must be evidence of company registration under the Company and Allied Matters Act (CAMA) and relevant professional/regulatory bodies.
(2) Bidders are to submit the following:
(a) Company profile details, to include registered address, functional email address, telephone numbers, and point of contact for the organization.
(b) Evidence of three similar completed projects.
(c) Evidence of three projects completed in a similar location.
(d) 3-year audited account/tax clearance.
(e) Evidence of financial capability supported by 6months of company bank statements.
(f) Affidavit on company integrity to demonstrate that none of the officials of intended State Ministry of Health, State PHCDA, and the intended Local Government, is a director, shareholder, or has any fiduciary relationship with the company.
(3) Interested companies should send all documents listed above, both hard and soft copy within two weeks of the date of this publication (not later than 3 November, 2022) to

This Invitation to Tender shall not attract any costs to the Private Sector Health Alliance of Nigeria on account of responses from intending bidders, and the process can be terminated at any time without any explanation if deemed necessary by the Private Sector Health Alliance of Nigeria.
