REQUEST FOR QUOTATION
RFP REFERENCE NUMBER: NUMBER IHP-ACO/RFQ/FY23/002
TITLE: “SUPPLIES AND INSTALLATION OF SOLAR PANEL, INVERTER, AND ACCESSORIES TO GENERAL HOSPITAL AND SECONDARY HEALTH FACILITIES IN BAUCHI, KEBBI, SOKOTO, EBONYI, AND FCT”
1. Overview
USAID’s Integrated Health Program (IHP) would like to invite eligible firms to submit offers for the Supplies and Installation of Solar Panel, Inverters, and Accessories to PHC Centre in Bauchi, Kebbi, Sokoto, Ebonyi, and FCT
The Vendors must demonstrate capacity, based on technical expertise, management capability, and financial resources.
- Palladium intends to award Purchase Order Agreement (contract) to the lowest responsive and responsible bidder.
RFQ Submission Requirements/Evaluation:
- Official quotation signed, with unit cost/goods, installation, and transportation cost indicated, dated, and stamped (where available) by an authorized representative of the offeror.
- Copy of the Company’s CAC Registration Certificate
- Copy of the Company’s Tax Identification Number (TIN) and Tax Clearance Certificate for 2019, 2020, and 2021
- Evidence of Health Insurance Policy uptake for all employees (for five employees and above, including the directors of the business)
- Copy of the Company’s Profile
- Evidence of doing a similar work within the last 3 years
- Delivery period if an order is placed after signing the Purchase Order.
- Payment Terms. (Note: Palladium does not make advance payments)
- Statement of Warranty Period.
- The Company must have a UEI number or obtain https://sam.gov/content/entity-registration one within 5 days of being notified of selection.

Issuance of this RFQ does not constitute an award commitment, nor does it commit Palladium to pay for costs incurred in the preparation and submission.
This submission of the quotation is on 7th July, 2023 between 1 pm-5 pm with the subject “Supply of Laptops and Accessories” quotation submitted before or after the submission date & time will be disqualified
