INVITATION TO TENDER FOR THE SUPPLY OF AMBULANCE AND HOSPITAL EQUIPMENT
The Management of UNIOSUN Teaching Hospital, Osogbo hereby invites interested/reputable companies to tender for the supply of the following:
Hospital Ambulance
1. Toyota Hiace Ambulance: 2020 Model
2. Toyota Sienna Ambulance: 2006 Model
Description:
The Ambulances must be equipped with Auto Load Stretcher, Medical tools including: Immobilization Tools, monitoring tools and defibrillation tools, infection control tools and injury Prevention Tools.
Equipment:
1. Semi Auto Chemistry Analyzer (Midray BA-88A)
2. Three (3) parts diff Hematology Analyzer
3. Microbiology Incubator
4. Hormonal Analyzer
5. Mobile Electrocardiogram– GE VIVID T9
6. ECG– Biocare 12 Channels.
7. Continuous Positive Airway Pressure (CPAP) Machine.
Eligibility for considerations
1. Certificate of incorporation
2. Company Tax Clearance
3. Personal Tax Clearance
4. Vat registration certificate.
5. Registration with Business Premises (Osun)
6. Evidence of registration with UNIOSUN Teaching Hospital, Osogbo.
7. Letter from Bank or Bank Statement
8. Memo and Article of Association Limited, Companies)
9. Audited account (Limited, Companies)
10. Registration of certificate with Osun State Bureau of Public Procurement.
11. List of ongoing and present project where applicable
12. Tenders fees– N100,000 (non-refundable)
Please Note:
1. UNIOSUN Teaching Hospital reserves the right to verify claims made by the supplier.
2. Submission of tender document does not commit UNIOSUN Teaching Hospital Osogbo to awarding contract to a tender.
3. Interested bidders should submit the type of the vehicles and equipment with the quotations at no cost to our establishment.
4. Interested companies who satisfy the above conditions should visit office of the Chief Medical Director for details.
5. All documents which must be acknowledge “received” should be in six (6) copies in sealed envelopes and addressed to:

6. All tender documents are to be submitted to the office of the Chief Medical Director latest by two weeks from the date of this publication.
