INVITATION TO TENDER FOR THE RECONSTRUCTION OF COMPREHENSIVE HEALTH CENTER, IKOSI ISHERI LOCAL COUNCIL DEVELOPMENT AREA, LAGOS
1.0 INTRODUCTION
The Ikosi isheri LCDA is seeking bid from a reputable and competent companies with good track record of performance and experience to tender for the reconstruction of comprehensive health center in Ikosi Isheri local council development area, Lagos.
2.0 PROJECT TENDER
2.01 RECONSTRUCTION OF COMPREHENSIVE HEALTH CENTER IN IKOSI ISHERI LOCAL COUNCIL DEVELOPMENT AREA.

4.0 ELIGIBILITY CRITERIA
Interested bidder(s) for above projects are required to provide the following bidding requirements for the projects. All bids must be accompanied with following;
i) Evidence of certificate of incorporation with the Corporate Affairs Commission [CAC] including form CAC1, and CAC2, and CAC7
ii) Evidence of company income tax clearance certificate for the last three years valid till 31st December 2023, with minimum average annual turnover of N100,000,000.00
iii) sworn affidavit
- Company’s audited accounts for the last 3 [three] years [2020, 2021, & 2022].
- Detailed company profile with the curriculum vitae of key staff to be deployed for the project, including copies of their academic and professional qualifications [COREN, ARCON, CORBON, QSRBN etc].
- Verifiable documentary evidence of at least three [3] similar jobs executed in the last five [5] years including letters of awards, valuation certificates, jobs completion certificates and photographs of the projects.
- For joint venture /partnership, Memorandum of Understanding [MOU] should be provided indicating among others the lead partner [CAC, tax clearance certificate, pension clearance certificate, ITF compliance certificate, NSITF clearance certificate, IRR & sworn affidavit are compulsory for each partner.
5.O COLLECTION OF TENDER DOCUMENTS
Interested companies are to collect the Standard Bidding Document [SBD] from the procurement unit, 3rd floor, Ikosi Isheri LCDA headquarters , Ikosi road, ketu Lagos. Payment of a non-refundable tender fee of ₦100,000 payment into ikosi Isheri LCDA revenue account 1012648914 UBA through any deposit money bank
6.0 SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit two hard copies each of the technical and financial bids packaged seperately in sealed envelopes and clearly marked technical bid, and financial bid, put the two seal envelopes together in a bigger sealed envelope addressed, head of procurement unit.
7.0 OPENING OF BID
The technical and financial bids will be opened immediately after the deadline (5th of December, 2023) for the submission at 11:00am in Adebayo Oshinowo hall, Ikosi Isheri LCDA, in the presence of the bidders or their representatives. Please ensure that you sign the bid submission register on 3rd floor, procurement unit will not be held liable for misplaced or wrongly submitted bids further requisite information required can be obtained from the procurement unit, Ikosi Isheri LCDA.
8.0 GENERAL INFORMATION
i. Bids must be in English language and signed by an official authorized by the bidder
ii. Bids submitted after the deadline for submission would be returned un-opened
iii. All costs will be borne by the bidders
iv. Ikosi-Isheri LCDA is bound to pre–qualify any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 46 of the Lagos state public procurement ACT 2021.
