EXPRESSION OF INTEREST FOR PRE-QUALIFICATION OF VENDORS FOR PARTICIPATION IN LIMITED COMPETITIVE PROCUREMENT ACTIVITIES
A. INTRODUCTION
Family Health International (FHI360) is not-for-profit human development organization dedicated to improving lives in lasting ways by advancing integrated, locally driven solutions. Our staff includes expert in health, education, nutrition, environmental, economic development, civil society, gender equality, youth, research, and technology, creating a unique mix of capabilities to address today’s interrelated development challenges. FHI360 is implementing the USAID’s Bureau of Humanitarian Assistant (BHA), Strengthening Humanitarian Assistance and Building Community Resilience in North-East Nigeria (NE_CORE) to provide acute needs of internally displaced persons (IDPs) in Borno State through the provision of integrated health and nutrition, protection, and WASH programming.
FHI360’s Borno State Office is therefore seeking to prequalify interested companies/firms/organizations that met the below prequalification requirement (Section B). Successful applicants shall have the opportunity to participate in our subsequent limited competitive procurement method.

C. PREQUALIFICATION REQUIREMENT
Please read this section carefully and be sure that all requested information is included in your submission. Evaluators will neither make assumptions nor draw inferences regarding missing or incomplete information. The submission of incomplete information could result in the rejection of proposal.
The minimum eligibility criteria to participate in the pre-qualification process shall be as under-listed and should be submitted in a sealed envelope.
This must be submitted in this manner:
i. Detailed Company Profile with registered addresses, functional email, telephone numbers and point of contact for the organization.
ii. Evidence of company statutory registration with Corporate Affairs Commission, including form CAC7 & CAC2 and any other business registration documents.
iii. Tax Clearance Certificate for 2021, 2022 and 2023
iv. Viable list of clients, including International Non- Governmental Organizations in Borno State including LGAS, other reputable organization equal in scope and Government.
v. Evidence of possession of experience in the line of field/category you are seeking prequalification. The evidence should include copies of completed Job Orders and Proof of Deliveries (Delivery Notes or Job Completion Notification/certificate vi. Evidence of Financial Capability (Latest Audited Accounts).
vii. Bank Reference Letter duly addressed to FHI360 Prequalification Committee
viii. Evidence of Registration with Professional bodies, Regulatory Bodies, Manufacturers and/or Certificate of Authorized ix. Any other relevant supporting document that could aid your application.
D. EVALUATION CRITERIA:
Stage one: Conformity to the requirement – Submissions must have all documents listed in Section B above. Evaluators will not make inference and assumption for incomplete information or document.
Stage two: Review of the evidence of experience(s) to ensure it matches the category of service applied for. Successful debarment search using UN, OFAC and SAM Searches
Stage Three: Successful physical verification and Reference Check.
E. GUIDANCE ON SUBMISSION:
Applicants are requested to submit the prequalification requirement document as guided in schedule below:

Interested applicants that require clarification should submit their query(ies) to NE_CORE_Procurement@fhi360.org not later than 5.30pm on Wednesday 29th May 2024
