INVITATION FOR PRE-QUALIFICATION FOR PROJECTS EXECUTION

1.0 INTRODUCTION
The Alex Ekwueme Federal University Teaching Hospital Abakaliki (AEFUTHA) is a tertiary health institution dedicated to the provision of quality, accessible and affordable healthcare services; and effective training and research. As part of its mandate to provide accessible and affordable healthcare services, the AEFUTHA intends to undertake the procurement of works/services under its 2024 capital appropriation.

2.0 SCOPE OF WORK

3.0 ELIGIBILITY REQUIREMENTS
Competent and interested companies who have the requisite experience and desire to execute the projects are required to submit the following documents with verifiable evidences for assessment and consideration:
a. Evidence of certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including forms CAC1.1 or CAC2 and CAC7;
b. Evidence of Company’s Income Tax Clearance Certificate for the Last three (3) years valid till 31st December, 2024;
c. Evidence of pension Clearance Certificate for the Last three (3) years (this requirement is only applicable to bidders whose number of staffs is 15 and above); receipt of payment will not be accepted.
d. Evidence of Industrial Training Fund (ITF) Compliance Certificate Valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is ₦50Million and above);
e. Evidence of Nigerian social Insurance Trust Fund (NSITF) Clearance Certificate Valid till 31st December 2024;
f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31/12/2024 or valid Certificate issued by BPP;
g. Sworn Affidavit:

• Disclosing whether or not any officer of the relevant committees of the Alex Ekwueme Federal university teaching hospital Abakaliki or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
• That no Director has been convicted in any country for any criminal offence relating to fraud of financial impropriety or criminal misinterpretation or falsification of facts relating to any matter;
• That the company is not in receivership, the subject of any form of insolvency or bankruptcy proceedings or the subject of any form of winding up petitions or proceedings;
• The claims made and information provided by the company in submission are true and correct in all ramifications;
• That the company does not have existing loan or financial liability with a bank, other financial institution or third party that is classified doubtful, non-performance, bad or whose repayment portion has been outstanding for the last four (4) months;

h. Company’s Audited Accounts for the last three (3) years -2021,2022,2023;
i. Reference letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company’s profile with the Curriculum Vitae of key Staff to be deployed for the project, including copies of their Academic/ Professional qualifications such as COREN, QSRBN, ARCON,CORBON, etc.
k. Verifiable documentary evidence of at least three (3) similar jobs (in size, nature and complexity), executed in the last five (5) years including Letters of awards, Valuation Certificates, Job Completion Certificates and photographs of the project;
l. List of Plants/Equipment with proof of Ownership/Lease Agreement;
m. For joint Venture/ Partnership, Memorandum of Understanding (MoU) indicating lead Partner should be provided (CAC, Tax Clearance Certificate, Pension Clearance Certificate, ITF Compliance Certificate, NSITF Clearance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner);

Note: All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Headed Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM NO.) and e-mail address. The Letter head Paper must bear the names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

4.0 SUBMISSION OF DOCUMENTS
Collection of standard bidding document for works:-
a. all standard bidding documents are to be collected from the head, procurement, procurement unit of Alex Ekwueme Federal University teaching hospital Abakaliki, (AEFUTHA) Ebonyi state at the 1st floor, Admin block, AEFUTHA 1 on evidence of payment of a non-refundable tender fee of ten thousand naira (10,000.00) only per lot issued in favor of AEFUTHA, ebonyi state remita account in any commercial bank.
b. All bidders must fill the bid collection register after the collection of the bidding document.

5.0 SUBMISSION OF THE TENDER DOCUMENTS
a. All respondents should submit (2) hard copies each of the technical and financial bids with softcopy of financial bid only in MS Excel format, packaged separately In sealed envelopes and clearly marked as “Technical Bid” or “Financial Bid”. Marked with the company name, address and lot numbers of projects bided for

5.1 SUBMISSION OF EOI
Interested firms are to submit the expression of interest (EOI) for the lots, in three (3) hard copies each (one original & two copies. The EOI should be addressed as indicated above and submitted not later than 12noon Monday, 5th august, 2024
b. All bidders must fill the bid submission register while submitting their technical and financial tenders in the tender’s box in the procurement office, admin block 1st floor, AEFUTHA 1.
c. All documents should be addressed to: CHIEF MEDICAL DIRECTOR, ALEX EKWUEME FEDERAL UNIVERSITY TEACHING HOSPITAL (AEFUTHA) ABAKALIKI, EBONYI STATE

6.0 CLOSING/OPENING DATE AND TIME:
i. The closing date for the submission of documents for EOI is Monday 5th August, 2024 at 12 noon while for invitation to tender is Monday, 2nd September, 2024 at 12noon prompt.
ii. The EOI and Technical bid received will be opened immediately on Monday 5th august, 2024 and Monday, 2nd September, 2024 respectively at the hospital auditorium, AEFUTHA 1.
iii. In line with section 19(b) (i) & (ii) of the PPA 2007, all bidders or their representatives, relevant professional bodies and non-governmental organizations (NGOs) are invited to witness the opening exercise accordingly. Please contact the procurement department on 08036865574, 08062960382:

7.0 GENERAL INFORMATION
a. Bids/expression of interest (EOI) must be in English Language and signed by an official authorized by the bidder;
b. Bids /expression of interest (EOI) submitted after the deadline for submission would be returned un-opened;
c. Bidders should not bid for more than two (2) lots.
d. All costs will be borne by the bidders.
e. The Bids/expression of interest (EOI) will be opened in the presence of Bidders or their representatives, CSOs in the areas of Anti-Corruption and Transparency and Private Professional bodies; and will be covered by video recording; invitation link will be sent to bidder who chooses to join the bid opening online;
f. Only pre-qualified bidders will be invited at a later date for financial opening

h. The bidders are expected to adhere strictly to all instruction, as non-compliance will constitute a ground for disqualification.
i. All CAC, VAT, PENCOM, TAX, ITF, certificates may be referred to the security agencies, corporate affairs commission, federal inland revenue services (FIRS), national pension commission and other relevant bodies for verification;
j. The Alex Ekwueme Federal University teaching hospital, Abakiliki is not bound to pre-qualify any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with Section 28 of the Procurement Act, 2007
