  5. Registration of Business premises (Osun).
  6. Evidence of registration with UNIOSUN Teaching Hospital, Osogbo.
  7. Letter from Bank or bank statement.
  8. Memo and Article of Association (Limited, Companies).
  9. Audited Account (Limited, Companies0.
  10. Registration of certificate with Osun State bureau of Public Procurement.
  11. List of the ongoing and present project where applicable.
  12. Payment of the sum of Twenty thousand naira (N20,000.00) Only as tender Fee (Non-Refundable).

  1. UNIOSUN Teaching Hospital reserves the right to verify claims made by the supplier.
  2. Submission of tender document does not commit UNIOSUN Teaching Hospital Osogbo to awarding contract to a tender.
  3. Interested bidders should submit the type of the equipment with the quotations at no cost to our establishment.
  4. Interested companies who satisfy the above conditions should visit office of the Chief medical Director for details.
  5. All documents which must be acknowledged “received” should be in six (6) copies in sealed envelopes and addressed to the Chief Medical Director, UNIOSUN Teaching Hospital, Osogbo.
  6. All Tender documents are to be submitted to the office of the Chief Medical Director latest by one (1 week) from the date of this publication.
