The Management of Benue State University Teaching Hospital, Makurdi hereby invite reputable and qualified contractors to tender for Construction, supply of Medical Equipment, Plant and Machinery, Consumables and Supplies to the hospital.

ELIGIBILITY REQUIREMENTS
i. Evidence of certificate of incorporation issued by Corporate Affairs Commission (CAC) including forms CAC2 and CAC7 or CAC1.1
ii. Evidence of Company’s Income Tax Clearance Certificate for the last three (3) years valid 31st December, 2024
iii. Evidence of Pension Clearance Certificate valid till 31st December, 2024 (this requirement is only applicable to bidders whose number of staff is 15 and above)
iv. Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December 2024 (this requirement is only applicable for bidders whose number of staff is 5 and above)
v. Evidence of current Nigerian Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2024
vi. Evidence of current license issued by Nigerian Security and Civil Defence Corps (NSCDC) for the year 2024.
vii. A copy of current license obtained from appropriate regulatory body.
viii. Sworn affidavit:
a. Disclosing whether or not any officer of the Benue State University Teaching Hospital, the Benue State Procurement Commission or the bureau of public Procurement is a former or has any peculiar interest in the bidder and to confirm that all information presented in the bid are true and correct in all particulars.
b. That the company does not have any Director who has been convicted in any country for any criminal offence relating to fraud or financial impropriety or criminal misrepresentation or falsification of facts relating to any matter.
c. That the Company is not in receivership, the subject of any form of insolvency or bankruptcy proceeding or the subject form of winding up of petition or proceedings.
ix. Company’s Audited Accounts for the last three (3) years 2022, 2023, 2024.
x. Reference Letter from a reputable Commercial Banks in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed.
xi. Verifiable documentary evidence at least (3) similar services rendered in the last 5 years including Letters of Awards etc.
xii. Category Registration with the Procurement Department of Benue State University Teaching Hospital.
xiii. All documents for submission must be transmitted with a cover letter under the Company Letter head paper duly signed bearing amongst others, the Registration Number as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Numbers, Email Addresses.
All enquiries should be channelled through the Procurement Department, BSUTH Makurdi and submissions must be made within Twenty-One (21) days from the date of publication.
