REQUEST FOR PROPOSALS (RFP): SUPPLY OF VARIOUS EQUIPMENT

Introduction
Heart of Hope International Ministry (HoH) is a faith-based non-governmental organization committed to empowering the poor, under-served, persons with disabilities and marginalized communities in Nigeria through inclusive education, healthcare, WASH (Water, Sanitation and Hygiene), and humanitarian services.
HoH hereby invites suitably qualified and experienced vendors with a proven track record to submit proposals for the supply of assistive devices, inclusive health facility upgrades, and start-up packs, as detailed below.

G. Bilateral Crutches (Armpit and Elbow)
2. Inclusive Health Facilities
A. Reconstruction and upgrading of Primary Health Centres (PHCs) for inclusive access
B. Construction of toilet ramps and installation of rails to enhance accessibility
3. Start-Up Packs
A. Sewing Machines
B. Hair Dryers & Accessories
C. Shoe Filling Machines & Accessories
D. Generators
E. Knitting Machines & Accessories
F. Barbing Clippers & Accessories
Submission Requirements
1. Expression of Interest (EOI) Letter
2. Certificate of Incorporation (CAC Registration)
3. Company/Organization Profile
4. Valid Tax Identification Number (TIN) and latest Tax Clearance Certificate
5. Evidence of similar or related previous supplies (e.g., Purchase order, Contracts, or Completion Certificates)
6. Any other relevant supporting documentation
Method of Submission

Alternatively, hard copies may be delivered in a sealed envelope to: The Procurement Unit, Heart of Hope International Ministry, Dandyes Chugwi, Vom, Jos South, Plateau State.
Submission Deadline: 31st October 2025, by 11:59 p.m.
