INVITATION FOR TECHNICAL AND FINANCIAL BIDS FOR THE IMPLEMENTATION OF 2019 APPROPRIATED CAPITAL PROJECT
In compliance with the Procurement Act 2007, the Federal Medical Centre Abeokuta hereby invites reputable and competent firms with relevant experience and good track records to submit Technical and Financial Bids for the implementation of 2019 Capital Projects:

2. ELIGIBILITY REQUIREMENTS
For Financial Bids to be considered, prospective bidders must be technically qualified by demonstrating qualification in the following areas:
a. Evidence of Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC2 and CAC7
b. Evidence of Company Income Tax Clearance Certificate for the last three (3) years valid till 31 December, 2019; with minimum average annual turnover of N20 million
c. Evidence of current Pension Compliance Certificate valid till 31 December, 2019
d. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2019
e. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31 December, 2019
f. Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 3/1/2020 or valid Certificate issued by BPP.
g. Sworn Affidavit:
– disclosing whether or not any officer of the relevant committees of Federal Medical Centre, Abeokuta or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that
– all information presented in its bid are true and correct in all particulars that neither the company nor its director is undergoing any form of receivership, insolvency or bankruptcy
h. Company’s Audited Accounts for the last three (3) years- 201620172018
i. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria; indicating willingness to provide credit facility for the execution of the project when needed;
j. Company Profile with the Curriculum Vitae of Key Staff to be deployed for the project including copies of their Academic/Professional qualifications in case of construction: COREN, OSBORN, IRON, CARBON etc.):
k. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and Photographs of the projects
l. List of Plant/Equipment with proof of Ownership/Lease (where applicable);
m. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner)
n. Only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorized representatives should bid for the vehicles a Letter of consent to allow for verification of all the documents submitted including CAC Pension, ITF, NSITF etc.
o. ALL documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm (xi) A Sworn Affidavit that no officer of Federal Medical Centre Abeokuta or the Bureau of Public Procurement (BPP) is a former or present Director, shareholder of the Company or has any pecuniary interest in the firm and also to confirm the authenticity of all documents submitted and
Failure to submit any of the listed documents may be sufficient grounds for disqualification
3. COLLECTION OF TENDER DOCUMENTS
Tender documents are to be collected from Procurement Department Room 001, office of the Head of Procurement, HES Building Federal Medical Centre, Idi-Aba. Abeokuta between the hours of 8.00am and 4.00pm (Monday-Friday) upon presentation of authenticated payment details of non-refundable tender fee of N10,000.00 Ten Thousand Naira) only per Lot paid into the Federal Medical Centre, Idi-Aba, Abeokuta remita account in any reputable commercial bank in Nigeria.
4. SUBMISSION OF TECHNICAL AND FINANCIAL BIDS
i. Documents for both Technical and Financial Bids which should be delivered by hand only shall Be submitted separately in sealed envelopes and addressed to

FEDERAL MEDICAL CENTRE
ii. Completed TECHNICAL AND FINANCIAL BIDS shall be submitted in two (2) hard copies and one (1) electronic copy (non-rewritable disk/DVD) in two (2) different TECHNICAL BIDS AND FINANCIAL BIDS respectively with the company names and contact numbers written boldly on it and dropped in the bid box in Room 001 is the Office of the Medical Director, Federal Medical Centre, Idi-Aba, Abeokuta between the hours of 8.00am and 4.00pm daily (Monday – Friday) on or before 12.00 noon of Monday 11 November, 2019.
The company name, full address and contact phone number must be clearly written at the back of each envelope.
5. OPENING OF TECHNICAL BIDS
Only the technical bids shall be opened on the same day, Monday 11 November, 2019 at 12.00 noon immediately after the close of submission at the Cardio Renal Hall of Federal Medical Centre, Idi-Aba. Abeokuta. Representatives of respondent Companies, Non-Governmental Organizations, Civil Societies and the General Public are by this advertisement invited to witness the bid opening exercise.
i. Late submission of Technical and Financial Bids shall be rejected
ii. Bids submitted outside the prescribed procedure shall not be considered
iii. Only pre-qualified bidders at technical evaluation will be contacted at a later date for financial bids opening, while financial bids of m-successful bidders will be returned un-opened
