INVITATION TO TENDER FOR SUPPLY OF GOODS
The Federal Medical Centre, Keffi Nasarawa State in furtherance of its statutory responsibility/mandate intends to implement some Capital Projects through funding from the 2019 Budget Appropriation.

2. ELIGIBILITY REQUIREMENTS
Interested and competent firm must have the following minimum qualifications:
i. Evidence of Registration with Corporate Affairs Commission (CAC) including form CAC1.1;
ii. Evidence of company’s income Tax clearance certificate for the last three years valid till 31st December 2019 with minimum annual average turnover of N100 million.
iii. Evidence of Company’s Nigeria Social Insurance Trust Fund (NSITF) compliance certificate valid 31st December, 2019.
iv. PENCOM Compliance Certificate expiring December 31st 2019.
v. ITF compliance certificate expiring December 31st 2019.
vi. Evidence of registration on the National Database of Federal Contractors, Consultants and Service Providers by inclusion of Interim Registration Report (IRR) valid till January 31, 2020 or valid certificate issued by the BPP.
vii. Firm’s Audited account for the last three years 2016, 2017 & 2018):
viii. Company Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications
ix. Evidence of at least three (3) similar jobs in nature and complexity executed within the last five (5) years including letter of contract award and certificate of job completion and visuals.
x. Evidence of financial capability to execute the project by submission of reference letter from a reputable commercial bank in Nigeria indicating willingness to provide credit facility for the execution of the project when needed.
xi. Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria, for bidders of Lots A1 & B2.
xii. Submission of a Sworn Affidavit disclosing whether or not any officer of the relevant committee of Federal Medical Centre, KEFFI or the Bureau of Public Procurement is a former or present director shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars.
xiii. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR & Sworn Affidavit are compulsory for each JV partner).
xiv. For Motor Vehicles procurement only companies included in the National Automotive Design and Development Council’s approved list of local manufacturers of vehicles or their authorized representative should bid for the supply.
xv. All documents for submission must be transmitted with a covering letter Head Paper bearing amongst others, registration number (RC) as issued by the corporate Affairs Commission (CAC), Contacts address, telephone number (Preferable GSM No.), and e-mail address. The letterhead paper must bear the names and nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
3. COLLECTION OF TENDER DOCUMENTS
Prospective Contractor/Supplier is to collect bid documents from the Procurement Department at the address below after payment of non-refundable tender fee of N10, 000 per Lot to TSA

4. SUBMISSION/CLOSING OF TENDER
Complete details of prospective contractor/supply (functional contact address, phone number and emails) together with two completed copies of TENDER FOR FINANCIAL AND TECHNICAL
BID for the capital projects must be packed in separate envelopes and both should be packed into another third sealed envelope clearly marked “Tender for (put package no., Lot No and description of supply)”not later than 12 noon on Tuesday 17th December, 2019 and addressed to: The Medical Director/CEO, Federal Medical Centre, old Akwanga road, Keffi, Nasarawa State and dropped at the Procurement Unit, ground floor at the new Admin Complex in the appropriate tender box within official working Days/Hours (8am-4pm).
5. OPENING OF TECHNICAL BIDS
Only the technical bids shall be opened immediately after the deadline for submission at 12 noon on Tuesday, 17th December, 2019 at the conference hall of the new Administrative Building, Federal Medical Centre, old Akwanga Road, Keffi in the presence of all the Bidders and any interested member of the public while the financial bids would be kept unopened.
Bidders should sign the bid submission register in the procurement unit at ground floor of the new administrative building, Federal Medical Centre, old Akwanga Road, Keffi as FMC Keffi would not be held liable for misplaced or wrongly submitted bids. 6. OTHER INFORMATION
i. Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening, while the financial bids of un-successful bidders will be returned un-opened.
ii. This serves as a notice and invitation to all interested stakeholders to observe the opening(s)
iii. Any company that tender for more than one (1) Lot will be disqualified;
