INVITATION TO TENDER FOR THE EXECUTION OF 2019 CAPITAL PROJECTS
The Federal Medical Centre, Owerri in fulfilment of her mandate to provide Quality and Painless Health Care Services, intends to use her budgetary allocation for the Procurement of Works and Services towards achieving her mandate as appropriated by Federal Government in the 2019 Capital Budget.
Accordingly, the Federal Medical Centre, Owerri invites qualified, interested and eligible contractors to tender for the procurement of Works appropriated in the 2019 Capital Budget.
2. SCOPE OF WORKS
PROJECT: Construction of a Two (2) Storey Ward Block and Provision of Solar Power in the lie-in wards.
3. ELIGIBILITY REQUIREMENT AND QUALIFICATION CRITERIA
Interested bidders shall present the following documents in their technical Tender:
(a) Evidence of certificate of Incorporation with the Corporate Affairs Commission Including form CAC 2 and CAC 7.
(b) Evidence of current Pension Compliance certificate valid till 31st December, 2019
(c) Evidence of company Income Tax clearance certificate for the last three (3) years valid till 31st December, 2019.
(d) Evidence of current Industrial Training Fund (ITF) compliance certificate valid till 31st December, 2019.
(e) Evidence of Registration on the National Database of Federal Contractors, consultants and services providers by submission of Interim Registration report (IRR) expiring on 31/1/2020 or valid certificate issued by the BPP.
(f) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2019.
(g) Verifiable documentary evidence of at least three (3) similar jobs in the last five (5) years including letter of Awards, Valuation Certificates Job completion and photographs of the projects.
(h) Evidence of Financial capacity to execute the project by submission of Reference reputable Commercial Bank to Nigeria, indicating willingness to provide credit facility for the execution of the projects when needed.
(i) Company’s Audited Accounts for the last three (3) years, 2016, 2017 and 2018
SWORN AFFIDAVIT THAT:
firm is not bankrupt
none of the Directors is an ex- convict or bankrupt;
disclosing whether or not any officer of the relevant committees of the (state name of the procuring entity, should NOT include the name of the Parent Ministry in a case you are a Parastatal or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that ail information presented in its bid are true and correct in all particulars;
that the Company would comply to Local Content Policy;
(k) Company Profile with Curriculum Vitae of key staff to be deployed for the project including copies of their Academic /Professional qualification and registration with professional bodies (COREN. QSRBN, ARCON CORBON etc)
(l) List of Plants/Equipments with proof of ownership/lease.
(m) All documents for submission must be transmitted with a covering/forwarding letter under the Company/Firm’s letter Head paper bearing amongst ither, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contacted Address, Telephone number (preferably GSM No) and e-mail address. The letter head paper must bear the names and Nationalities of the Directors of the Company on the bottom of the page, duly signed by the authorized officer of the firm.
Note: All pages of document submitted must be authentication by the Company CEO or his/her representatives, failure will lead to disqualification.
4. COLLECTION OF TENDER DOCUMENTS
Interested bidders are to collect Standard Bidding Document from the Office of the Head, Procurement Unit, First floor of ABC (Admin.) Complex, Federal Medical Centre, Owerri on presentation of evidence of payment of a non-refundable sum of N10,000.00 ( ten thousand naira) only paid into the Federal Medical Centre. Owerri’s Remits account in any Commercial Bank.
5. SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit two (2) hard copies of the Technical and Financial Bids packaged separately in sealed envelopes and dearly marked as Technical Bid or “Financial Bid” with name of the Bidder, address and phone number. Put the two sealed envelopes together in a bigger sealed envelope with name of Bidder, address and phone number addressed to:

Federal Medical Centre,
And clearly marked with the name of the project and dropped in the designated Tender Box not later than 12:00 noon, Tuesday 22 October, 2019 at Procurement Unit Upstairs, ABC (Admin) Complex Federal Medical Centre, Owerri.
6. OPENING OF BIDS
The Technical Bids will be opened immediately after the deadline for submission at 12:00 noon, Tuesday 22nd October, 2019 at the Conference Room on the 1st floor (back) of the Diagnostic Centre in the presence of Bidders or their representatives, while the financial bids will be kept un-opened. Note that, the Centre will not be held liable for misplaced or wrongly submitted Bids.
7. GENERAL INFORMATION
(a) Bids must be in English language and signed by an official authorized by the bidder.
(b) Bids must be in Naira currency.
(c ) Bids submitted after the dateline for submission would be returned un-opened
(d) All costs will be borne by the bidder
(e) Only Technical qualified bidders will be contacted at a later date for financial bids opening, while financial bids of unsuccessful bidders will be returned un-opened.
(f) The Federal Medical Centre, Owerri reserves the right to annul the process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act 2007.
8. This advertisement shall not be construed as a commitment on the part of the Centre or shall it entitle tenderers to make any claims or seek any indemnity from the Centre by virtue of their response to this advertisement.
