INVITATION TO TENDER FOR THE EXECUTION OF 2019 CAPITAL PROJECTS
The Federal Medical Centre, Umuahia in fulfilment of her mandate to provide Quality and Qualitative Health Care Services, intends to use her budgetary allocation for the procurement of works, goods and services towards achieving her mandate as appropriated by the Federal Government in the 2019 Capital Budget.
Accordingly, the Federal Medical Centre, Umuahia invites qualified, interested and eligible Companies (Manufacturers, Suppliers, Contractors etc.) to tender for the following procurement of Goods. Works and services in the 2019 Capita Budget.
The scopes of this procurement are as follows:
(i) Constriction of 1.5km Internal Ring Road.
(ii) Construction of 2 Storey House Officers Building with flats.
(iii) Construction of Modern Accident & Emergency/Modern Intensive Care Unit
(i) Hospital Equipment and Furniture
(a) ENT Endoscopic Pack (Theatre)
(b) ENT Endoscopic Pack (Out Patient)
(c) Theatre Equipment
(d) Renal Equipment
(e) Office Furniture
(f) Paediatrics Equipment
C. PROJECT CONSULTANT
(i) Project Consultant for the Construction of 1,5km Internal Ring Road.
(ii) Project Consultant for the Construction of 2 Storey House Officers Building with Flats
(iii) Project Consultant for the Construction of Modern Accident & Emergency/Modern Intensive Care Unit
3. ELIGIBILITY REQUIREMENT AND QUALIFICATION CRITERIA
Interested bidders shall present the following documents in their technical
(a) Evidence of certificate of Incorporation with the Corporate Affairs Commission (CAC) including form CAC 2 and CAC 7
(b) Evidence of current Pension Compliance certificate valid till 31st December, 2019.
(c) Evidence of company Income Tax clearance certificate for the last three (3) years valid till 31st December, 2019.
(d) Evidence of current Industrial Training Fund (ITF) compliance certificate valid till 31st December, 2019.
(e) Evidence of Registration on the National Database of Federal Contractors, consultants and services providers by submission of Interim Registration report (IRR) expiring on 31/1/2020 or valid certificate issued by the BPP;
(f) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2019;
(g) Verifiable documentary evidence of at least three (3) similar jobs in the last five (5) years including letter of Awards, Valuation Certificates Job completion and photographs of the projects.
(h) Evidence of Financial capacity to execute the project by submission of Reference letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the projects when nodded;
(i) Company’s Audited Accounts for the last three (3) years 2016, 2017 and 2018;
(j) Sworn affidavit that:
– Firm is not bankrupt
– None of the Directors is an ex- convict or bankrupt;
– Disclosing whether or not any officer of the relevant committees of the (state
name of the procuring entity, should NOT Include the name of the Parent
Ministry in a case you are a Parastalal) or the Bureau of Public: Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in Its bid are true and correct in all particulars;
• that the Company would comply to Local Content Policy;
(k) Company Profile with Curriculum Vitae of key staff to be deployed lot the project, including copies of their Academic /Professional qualification and registration with professional bodies (COREN, OSRBN, ARCON, CORBON etc)
(l) List of Plants/Equipment with proof of ownership/lease
(m) All documents for submission must be transmitted with a covering/ Forwarding letter under the Company /Firm’s letter Head paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC). Contact Address, Telephone number (preferably GSM No) and e-mail address The letter head paper must bear the names and Nationalities of the Directors of the company on the bottom of the page, duly signed by the authorized officer of the firm NOTE: All pages of document submitted must be authenticated by the Company CEO or his/her representative, failure will lead to disqualification
4. COLLECTION OF TENDER DOCUMENTS
The bid document can be obtained from the procurement unit, Federal Medical Centre, Umuahia upon an evidence of payment of Ten Thousand Naira (N10,000.00) only non-refundable fee.
(5) SUBMISSION OF TENDER DOCUMENTS
(a) Prospective bidders are to submit two (2) hard copies of the Technical and Financial Bids packaged separately in sealed envelopes and clearly marked Technical Bid or “Financial Bid” with the name of the bidder, address and phone number Put the two sealed envelopes together in a bigger sealed envelope with name of bidder, address and phone number addressed to:

(b) Bid documents should be obtained within four (4) weeks of this publication and submitted not later than six (6) weeks (16 November, 2019) from the dale of this publication.
(c) The bid document should be deposited in the tender box specified for the project at the office of the Head Procurement. Federal Medical Centre, Umuahia. Company name and address should also be within at the reverse side of the envelope. Closing time for all submission is on or before 12 noon prompt Monday 16 November, 2019. Late submission shall not be entertained.
6. OPENING OF BIDS
The Technical Bids will be opened immediately after the deadline for submission at 12noon, Monday 18 November, 2019 at the Board Room 2nd Floor, New Administration Complex. All bidders or their representatives, relevant professional bodies and NGOs are invited to witness the public opening exercise accordingly.
7. GENERAL INFORMATION
(a) Bids must be in English Language and signed by an official authorized by the bidder.
(b) Bids must be in Naira Currency.
(c) Bids submitted after the deadline for submission would be returned un-opened.
(d) All costs will be borne by the bidder.
(e) Only Technical qualified bidders will be contracted at a later date for financial bids opening, while financial bids of un-successful bidders will be returned un-opened.
(f) The Federal Medical Centre, Umuahia reserve the right to annul the process at any time without incurring any liability in accordance with section 28 of the Public Procurement Act 2007.
(g) This advertisement shall not be construed as a commitment on the part of the Centre or shall it entitle tenderers to make any claims or seek any indemnity from the Centre by virtue of then response to the Advertisement.
(h) All CAC, PENCOM, ITF, NSITF and TAX certificates maybe referred to the Security Agencies, Corporate Affairs Commission, Federal Inland Revenue (FIRS). National Pension Commission and other relevant bodies (or verification Prospective Contractors should ensure they sign the register provided on submission of bid documents.
