INVITATION TO TENDER FOR 2019 FGN CAPITAL PROJECT
The Federal Medical Centre, Jalingo in compliance with Public Procurement Act, 2007 and extant Laws is desirous of engaging reputable, competent and qualified companies for the execution of her 2019 capital project as follows.
2.0 SCOPE OF WORKS

|Lot 3||Procurement of Utility Vehicle|
|Lot 4||Procurement of essential Medical Equipment|
|Lot 5||Lassa Fever Public Campaign end Control|
|Lot 6||Provision of Medical Outreach in Karim Lamido|
|Lot 7||Furnishing of Wards|
3.0 ELIGIBILITY REQUIREMENTS
(i) Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including Forms CAC2 andCAC7.
(ii) Evidence of Company Income’s Tax Clearance Certificate for the last three (3) years (2016, 2017 & 2018) valid till 31st December 2019.
(iii) Evidence of Current Pension Compliance Certificate valid tin 31st December, 2019.
(iv) Evidence of Current Industrial Training Fund (ITT) Compliance Certificate valid till 31st December, 2019.
(v) Evidence of Current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2019.
(vi) Evidence of Registration on National Database of federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 30/06/2019 or valid Certificate issued by BPP.
(vii) SWORN AFFIDAVIT:
disclosing whether or not any officer of the relevant committees of the Federal Medical Centre, Jalingo or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder, and to confirm that all information presented in its bid are true and correct in all particulars
that the company is not bankrupt in receivership or under liquidation or involved in any litigation which can potentially affect the company’s ability to effectively: discharge their responsibility if engaged.
(viii) Company’s Audited Accounts for the last three (3) years (2016, 2017 & 2018).
(ix) Evidence of Financial Capability to Execute the Project by submission of Reference Letter from a Reputable Commercial Bank in Nigeria, including willingness to provide credit facility for the execution of the project when needed.
(x) Company’s profile with the Curriculum Vitae of key staff to be deployed including copies of Academic/ Professional Certificates.
(xi) List of Plans /equipment with proof of Ownership/ lease (where applicable) will be an added advantage.
(xii) For motor vehicle’s procurement, only companies included in national automotive design and development council’s approved list of local manufacturers of vehicles or their authorized representatives should bid for the procurement.
(xiii) Letter of Authorization from the Original Equipment Manufacturers listed in the Approved Policy for Procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria, for bidders of Lot 3;
(xiv) All documents for submission must be transmitted with a Covering/forwarding letter under the Company/firm’s Letter Head Paper bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No ’), and e-mail address. The Letter Head Paper must bear the Names and Nationalities of the Directors of the Company at the Bottom of the page, duly signed by the authorized officer of the firm.
4.0 COLLECTION OF TENDER DOCUMENTS (LOTS 1-4)
Interested and qualified companies should collect relevant tender document from the office of the Head of Procurement RM1 Finance and Account Block, Federal Medical Centre, Jalingo, Taraba State upon the presentation of evidence of payment of a non- refundable Tender fee of N10.000 in favour of The Federal Medical Centre, Jalingo TSA.
NOTE: Interested bidders for Consultancy Services are not expected to pay at this stage until when shortlisted for Request for Proposal.
5.0 SUBMISSION OF TENDER DOCUMENTS (LOTS 1-4)
Prospective bidders are to submit bid for each of the desired Lot Two (2) hard copies of the technical and financial bids with soft copy of financial bid only in MS Excel format package separately in sealed envelopes and dearly marked as “Technical Bid” of “Financial Bid” Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to: The Medical Director, Federal Medical Centre, Jalingo, Taraba State.
Furthermore, the reverse of each sealed envelope should have the name and address of the bidder and drop in the designated Tender Box in the above address not later than, 12:09 Noon on Monday 15 July, 2019.
6.0 SUBMISSION OF EOI DOCUMENTS (LOTS)
Interested firms are to submit two (2) bound of Expression of interest (Eol) documents separated by dividers and arranged as indicated above. The Documents submitted in a sealed envelope and addressed to The Medical Director, Federal Medical Centre, Jalingo, Taraba Stat, and clearly marked with the name of the project and lot number. Furthermore, the reverse of each sealed envelope should have the name and address or the bidder and drop in designated Tender Box not later than 12:00 Noon on Monday, 24 June, 2019.
7.0 OPENING OF EOls/BIDs:
(a) Expression of Interest will be opened at 12:00 Noon, Monday, 24 June. 2019 at the boardroom of the Medical Director, FMC. Jalingo.
(b) The technical bids will be opened immediately after the deadline for submission 12:00 Noon on Monday, 15 July, 2019 at the boardroom of the Medical Director, FMC Jalingo the presence of bidders or their representatives, while the Financial bids will be kept un-opened. Please, ensure that you sign the Submission Register in the office of the Medical Director, as the Hospital will not be held liable for misplaced or wrongly submitted bids. 8.0 ADDITIONAL INFORMATION:
(a) Any Bid submitted later than the deadline for the submission of bids will be returned un-opened.
(b) Any fake unverifiable document/certificate shall be submitted to relevant prosecuting bodies for further action after due diligence.
(c) This invitation shall not be construed as commitment to award the contracts) in favour of any company.
(d) Bidders that does not adhere to instruction shall risk been disqualified from the process
(e) Lots 1-4: Only pre-qualified bidders at technical evaluation will be Invited at a later date for financial bids opening, while the financial bids of unsuccessful bidders will be returned un opened;
(f) Lot 5: Only shortlisted bidders will be contacted at a later date for collection of Request for proposals
The FMC Jalingo is not bound to shortlist any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007
