INVITATION FOR EXPRESSION OF INTEREST FOR THE EXECUTION OF 2019 MEDICAL OUTREACH PROGRAMME
1. INTRODUCTION
The Federal Medical Centre, Lokoja, hereby invites reputable and interested contractors and experience and good records of accomplishment of performance to submit bids for the following project:
2. SCOPE OF SERVICES
Medical Outreach at Kabba-Bunu/ljumu Federal Constituency Kogi State.
3. ELIGIBILITY REQUIREMENTS
The following are the requirements that should be submitted by interested contractors: –
a. Evidence of Certificate of Incorporation with the Corporate Affairs Commission (CAC) including form CAC2 and CAC7 or Business Name where applicable:
b. Evidence of Company Income Tax Clearance Certificate (or Personal Income Tax Clearance Certificate of all the Partners for the last three (3) years (2016, 2017 and 2018) valid till 31st December, 2019
c. Evidence of current Pension Compliance Certificate valid till 31st December 2019;
d. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2019;
e. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2019
f. Evidence of registration on the National Database of Federal Contractors Consultants and Service providers by submission of Interim Registration Report (IRR) expiring on 31/1/2020 or valid Certificate issued by BPP
g. Company’s Audited Accounts for the last three (3) years- 20162017 & 2018;
h. Evidence of financial capability to execute the project by submission of Reference Letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
i. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications:
j. Verified documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards and Job Completion Certificates;
k. Evidence of Firm’s Registration with the Medical & Dental Council of Nigeria, or other related professional bodies);
l. Sworn Affidavit:
– disclosing whether or not any officer of the relevant committees of the Federal Medical Centre, Lokoja or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars
– that the Director(s) of the company has/have never been convicted by any Court of Law;
– that the company is not in receivership, that neither the company nor any of the Director is bankrupt;
– All documents for submission must be transmitted with a Covering/Forwarding under the company/firm’s Letterhead Paper bearing amongst others, the Registration Number (RC) is issued by the Corporate Affairs Commission (CAC) Contact Address, Telephone Number (preferably GSM No.); and email address, Directors of the Company at the bottom of the page and duly signed by the authorized officer of the firm.
4. SUBMISSION OF EOI DOCUMENTS
Prospective bidders are to submit bid in two (2) spiral bounded hard copies each of the completed pre-qualification document with full pagination and enclosed in an envelope, sealed and clearly marked with project name, at the Top right comer of the envelope addressed to:

Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop in the designated Tender Box positioned at the entrance to the Board room located in the Central Administrative block not later than 12:00 noon on Monday, 16th December, 2019. Please, ensure that you sign the Bid Submission Register in the
Procurement Unit office located in the Central Administrative Block, as the Federal Medical Centre, Lokoja, will not be held liable for misplaced or wrongly submitted bids.
5. OPENING OF EOI DOCUMENTS
The prequalification documents will be opened immediately after the deadline for submission at 12:00 noon on Monday, 16th December, 2019, in the Board Room Federal Medical Centre, No. 1 Salihu Ibrahim Way, Lokoja, Kogi State, in the presence of bidders or their representatives. 6. GENERAL INFORMATION
a. Bids must in English Language and signed by an official authorised by the bidder:
b. Only shortlisted firms shall be invited at a later date for collection of Request for Proposals.
c. Bids submitted after the deadline for submission would be returned un-opened;
d. All costs will be borne by the bidders;
e. The Federal Medical Centre, Lokoja, is not bound to shortlist any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act 2007.
