NATIONAL TUBERCULOSIS, LEPROSY AND BURULI-ULCER CONTROL PROGRAMME
INVITATION FOR EXPRESSION OF INTEREST
The National Tuberculosis and Leprosy Control Programme (NTBLCP) is a division in the Department of Public Health, Federal Ministry of Health (FMOH) and is saddled with the responsibility of controlling Tuberculosis in Nigeria. The NTBLCP received a Global Fund grant for Tuberculosis for the year 2019 to 2020 and has been made a Principal Recipient. The role in the grant is to coordinate all public sector Global Fund implementation and provide national guidance.
2.0 INVITATION FOR EXPRESSION OF INTEREST:
NTBLCP is soliciting for interested and reputable Services providers, Contractors, Suppliers and Organizations with relevant experience in the following services during the grant implementation period:

|LOT NO 2||Provision of services for Tuberculosis case finding activities among Nomads in Adamawa, Taraba and Gombe States.|
|LOT NO 3||Provision of services for TB case finding activities among Internally displaced persons in Adamawa, Yobe, and Borno State.|
|LOT NO 4||Provision of 3rd party logistics services in six geopolitical zones of the country under the following categories;|
|Category A||South West Zone consisting of Lagos, Ekiti, Oyo, Osun, Ogun and Ondo|
|Category B||South East Zone consisting of Abia, Anambra, Ebonyi, Enugu and Imo|
|Category C||South South Zone consisting of Akwa lbom, Bayelsa, Cross River, Delta, Edo and Rivers|
|Category D||North East Zone consisting of Adamawa, Bauchi, Borno, Gombe, Taraba and Yobe|
|Category E||North West Zone consisting of Kaduna, Kano, Katsina, Kebbi, Jigawa, Sokoto and Zamfara|
|Category F||Benue, FCT, Kogi, Kwara, Nassarawa, Niger and Plateau|
|LOT NO 5||Provision of services for clearing and forwarding of Tuberculosis commodities.|
|LOT NO 6||Testing of persons with bearing impairment and supply of hearing aid.|
|LOT NO 7||Printing services|
|LOT NO 8||Supply of IT equipment and consumables|
|LOT NO 9||Supply of office furniture|
|LOT NO 10||Repairs and maintenance of Vehicles|
|LOTNO 11||Provision of Security services|
|LOT NO 12||Repairs and maintenance of Generator|
|LOT NO 13||Provision of services for insurance|
|LOT NO 14||Supply of Laboratory consumables|
|LOT No 15||Provision of courier services|
1.0 GENERAL ELIGIBILITY CRITERIA
(i) Evidence of company registration (certificate of incorporation of the company)
(ii) Certified True Copy of Memorandum and Article of Association with Corporate Affairs Commission or similar government regulatory institution in your country of operation.
(iii) Duly Certified Company Audited Accounts for the last 3 years (2016, 2017 and 2018)
(iv) Evidence of Tax payment (current tax clearance certificate for the last three years (2016, 2017 and 2018) or tax exemption certificate.
(v) Verifiable evidence of completing similar jobs successfully.
(vi) Letter of financial Capability from a reputable commercial bank
(vi) All bids must be accompanied with a sworn affidavit disclosing whether or not any officer of NTBLCP is a former or present Director, Shareholder or has any pecuniary interest in the bidder and confirming that all information presented in its bid are due and correct in all particulars.
(viii) Submission of a comprehensive company Profile.
(ix) Evidence of current Industrial Training Fund (1TF) Compliance Certificate valid till 31st December, 2018;
(x) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2018;
(xi) Firms submitting as joint venture (JVs) should include memorandum of understanding (MOU) indicating the responsibilities and duties of the individual firms constituting the JV as well as names’ and skills of key staffs under each company in the JV.
(xii) A signed cover letter is to be submitted with the bid documents.
(xiii) Evidence of current pension compliance certificate.
(xiv) Evidence of registration with pharmacist council of Nigeria (PCN) for LOT N0 4and 14
(xv) Letter of Authorization from Original Equipment Manufacturers for LOT NO 8
(xvi) Registration and Certification by National Insurance Commission (NAICOM) for LOT No 13
Only Selected organizations from this EOI will be invited for the submission of detailed Technical and Financial proposals.
2.0 APPLICATION INSTRUCTION:
(a) One company can apply separately for up to two (2) different LOTS however only one application is allowed from each bidder for each lot.
(b) Completed EOI and other documents must be in English Language and submitted in four (4) copies (one original & three copies) in a sealed envelope clearly marked at the topmost right comer indicating the LOT number and NTBLCP EXPRESSION OF INTEREST” with the Name, Address of the firm(s) boldly written at the back of the envelope, with acknowledgment to NTBLCP office at the address below.

(c) All EOI documents must be sequentially arranged according to the EOI requirements above and failure to comply with this or any of the advertised submission guideline will lead to outright disqualification.
(b) Request for further clarification and enquiry prior to close of submission should be made to firstname.lastname@example.org Any bid submission via email will be disqualified.
3.0 CLOSING DATE:
The deadline for submission is by 12noon/Tuesday, June 18, 2019 (local time). Any submission after the stipulated time will be disqualified.
4.0 OPENING OF EOI
The EOI will be opened immediately after the deadline for submission by 12 noon Tuesday, June 18, 2019 at the NTBLCP conference room, in the presence of bidders or their representatives
(a) Late submission will note be accepted
(b) NTBLCP shall verify all documents and claim made and will disqualify bidders with falsified information.
(c) This advertisement shall not be construed as a commitment on the part of NTBLCP to award a contract or shall it entitle bidders to claim any indemnity from NTBLCP.
(d) NTBLCP reserves the right to annul the process without incurring any liability.
(e) All cost related to the EOI shall be borne bidders.
