INVITATION TO TENDER
National Ear Centre, Kaduna was established to offer quality and reliable ear, Nose and Throat specialist services based on international best practices, standards and to promote deafness prevention activities in the communities. In compliance with Section 25(ii) of the Public Procurement Act 2007, the National Ear Care Centre Kaduna hereby invites interested, competent and reputable bidders with relevant experience to submit technical and financial bids for the following projects:
2. SCOPE OF WORKS/SUPPLIES
|LOT 1||PROCUREMENT OF TEACHING AIDS AND INSTRUMENTS|
|LOT 2||CONSTRUCTION AND FURNISHING OF LECTURE THEATRE|
|LOT 3||RENOVATIONS OF ENT BLOCK, SCHOOL OF POST BASIC ORL & STUDENT HOSTEL|
3. ELIGIBILITY REQUIREMENTS
i. Evidence of certificate of Incorporation with the Corporate Affairs Commission (CAC) including Memorandum and Article of Association, Forms CAC2 and CAC7
ii. Evidence of current CAC Annual Returns by inclusion of CAC’s Letter of Acknowledge of filling Annual return (s)
iii. Evidence of company income tax clearance certificates for the last three years valid till 31st December, 2019
iv. Evidence of current Pension Compliance Certificate valid till 31st December, 2019
v. Evidence of current Industrial Training Fund (ITF) compliance certificate valid till 31st December, 2019
vi. Evidence of current Nigeria Social insurance Trust Fund (NSITF) Compliance certificate valid till 31st December, 2019
vii. Evidence of registration on the National Database of Federal Contractors, Consultants and service providers by submission of Interim registration Report (IRR) expiring on 31/01/2020 valid Certificate issued by the Bureau of Public Procurement (BPP)
viii. Sworn affidavit disclosing whether or not any officer of the relevant Committees of the National Ear Care Centre, Kaduna or the Bureau of Public Procurement is a former or present Director, Shareholder or has any interest in any bidder that the company is not in receivership or under liquidation or involved in any litigation which can potentially affect the company’s ability effectively discharge their responsibility if engaged and to confirm that all information presented in its bid are true and correct in all particulars.
ix. Evidence of previous experience on supply of teaching aid, insrument5s, medical equipment and contraction activities by inclusion of a minimum of three (3) Letters of Awards & Job completion Certificates for such activity;
x. Detailed Company’s profile including Curriculum Vitae of key personnel’s to be deployed for the project and copies of their Academic/Professional qualifications: COREN, QSRBN, ARCON, CORBON
xi. Company’s Audited accounts for the last three (3) three ears – 2016, 2017 & 2018
xii. Evidence of financial Capacity to execute the project by submission of reference Letter from a reputable Commercial Bank willingness to provide credit facility for the execution of the project when needed.
xiii. Bidders bidding for Lots 2&3 must be companies duly incorporated as construction companies by Corporate Affairs Commission and
xiv. All bidding documents for submission must be transmitted with a Covering/Forwarding Letter under the company’s Letter Head paper bearing amongst other, the Registration Number (RC), as issued by the Corporate Affairs Commission (CAC), contact Address, Telephone Numbers (preferable GSM No.) and valid e-mail address and official company’s address. The letterhead Paper must bear the Names, Nationalities and telephone number of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm
4. COLLECTION OF TENDER DOCUMENTS
Interested companies are to collect the standard Bidding Document (SBD) from the Procurement Unit (ROOM B9) of National Ear Care Centre Kaduna with the payment of non-refundable tender fee of N10, 000 per Lot, paid into the National Ear Centre Kaduna’s remita account.
5. SUBMISSION OF BIDS
The technical and financial bids shall be separate documents. Two (2) copies each of the technical and financial bid must be SEPERATELY packaged in sealed envelopes, clearly marked as either Financial or Technical bid, as appropriate and put together in SINGLE (bigger) envelope which should be marked CONTRACTOR NAME and address to: The office of head of procurement Unit (ROOM B9) National Ear Care Centre Kaduna No. 3, Golf Course Road/Independence Way Kaduna not later than 11:00 am Tuesday 24th September, 2019.
6. OPENING OF TECHNICAL BIDS
Only the technical bids will be opened immediately after the deadline for submission at 11:00 am on Tuesday 42th September, 2019 at the conference hall of National Ear Care Center Kaduna in the presence of bidders or their representatives, while the financial bids will be kept un-opened. Please ensure that you sign the Bid Submission Register in the office of the Procurement (ROOM B9) National Ear Care Center Kaduna No. 3, gold Course Road/Independence Way Kaduna, as the National Ear Centre Kaduna will not be held liable for misplaced or wrongly submitted bids.
7. GENERAL INFORMATION
i. The scope of supply above does not constitute conclusive description & specifications of items, detail description & specification shall be found in the Standard Bidding Documents
ii. Bidders should not bid for more than one (1) Lot
iii. Only bidders whose technical bids fulfill the eligibility requirements would have their financial bids considered. All enquiries should be directed to the address where bid documents were collected.
iv. Bids submitted after the deadline for submission would be returned un-opened
