The Obafemi Awolowo University Teaching Hospitals Complex was Appropriated Funds in the 2009 Capital Budget for the execution of its projects. Accordingly, in compliance with Public Procurement Act 2007, the Obafemi Awolowo University Teaching Hospital Complex hereby invites all experienced and competent suppliers to submit bids for the under listed Medical equipment project under 2019 Budget.


i.Supply of Radiology Equipment
ii.Supply of Orthopaedic Surgery and Traumatology Equipment
iii.Supply of Ophthalmology Equipment
iv.Supply of Cardiac and Cardiology Equipment
v.Supply of Dental Equipment

The execution of the Projects Listed above will be subject to Due Process Mechanism, which would be transparent to all Bidders who wish to participate.

a. Evidence of Certificate of Incorporation issued by the corporate affairs commission (CAC) including forms CAC2 and CAC7
b. Evidence of company incomes tax clearance certificate (or personal income tax certificate where applicable for the last three years valid till 31st December, 2019
c. Evidence of current pension compliance certificate valid till 31st December, 2019
d. Evidence of current industrial training fund (ITF) Compliance Certificate valid till 31st December, 2019
e. Evidence of registration of current Nigeria Social Insurance Trust Fund (NSITF) compliance certificate valid till 31st December, 2019
f. Evidence of registration on the national database of federal contractors, consultants and service providers y submission of interim registration report (IRR) expiring 0n 31/1/2020 or valid certificate issued by BPP
g. Sworn Affidavit disclosing whether or not any officer of the relevant committees of The Obafemi Awolowo University Teaching Hospitals Ile-Ife, Osun State or the bureau of public procurement is a former or present director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
h. Company’s audited accounts for the last three (3) years – 2016, 2017, 2018
i. Evidence of financial capability to execute the project by submission of reference letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Company profile with curriculum vitae of key staff to be deployed for the project, including copies of their academic/professional qualifications (in case of construction
k. Verifiable documentary evidence of at least three similar jobs executed in the last five years including letters of awards, valuation certificates, job completion certificates and photographs of the projects;
l. For joint venture/partnership, memorandum of understanding (MoU) should be provided (CAC, Tax clearance Certificate, pension compliance certificate, ITF Compliance certificate, NSITF Compliance certificate, IRR & Sworn Affidavit are compulsory for each JV partner)
m. Letter of Authorisation from original equipment manufacturers
n. All documents for submission must be transmitted with a covering/Forwarding letter under the company/Firms letter head paper bearing amongst others, the Registration number(RC) as issued y the corporate affairs commission (CAC) contact Address, telephone number (Preferably GSM No.) and email address. The letterhead paper must bear the names and nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorised officer of the firm.

Note: copy of Award Letters, Completion Certificates and final maintenance certificates where applicable is to be enclosed.

The Tender Documents can be collected from the office of the Director (procurement department) room 7, Behind Administrative Block, Phase 1, OAUTHC, ILE-IFE between working days 8.30am and 4pm daily from Monday, 12th August, 2019 on payment of a non-refundable tender fee of N10, 000.00 per lot paid into OAUTHC’s Remittal account. The photocopy of teller for this payment should be attached to the tender documents.

The tenderer shall enclose the original in one envelope and the copy of the tender in another envelope duly marking the envelope as original and copy. The two envelopes shall then be enclosed and sealed in one single outer envelope.

Completed Technical and Financial Tender documents must be neatly bound and sealed in separate envelopes clearly marled, at the top right corner of each of the envelopes with the title of project, e.g. Supply of Medical Equipment
Bearing a statement “DO NOT OPEN 12.00 noon on Tuesday 24th September 2019.

If all envelopes are not sealed and marked as required, OAUTHC will assume no responsibility for the misplacement or premature opening of the Tender.

The completed and sealed tender document should be deposited in the Tender Box in the Office of the Director (Procurement) Room 7, Behind Administrative Block, Phase 1 OAUTHC Ile-Ife not later than 12.00 noon on Tuesday, 24th September, 2019. – The OAUTHC, Ile-Ife will only recognize and correspond with only authorized officers of the tendering companies and not through individuals or agents acting on their behalf
