INVITATION TO TENDER
The Federal Government of Nigeria through the office of the Senior Special Assistant to the President on sustainable Development Goals (OSSAP-SDGs) and Bayelsa state Government Goals through the office of the special Adviser to the Governor, sustainable development goals (PSU-SDGs). From its approved budget for the fiscal year desires to carry out renovation of primary Health centers, supply and distribution of wheel chairs and crutches in the state under the CGS-SDGs 2018 project and intends to apply part of the budget for the payment of the contracts. Consequently, the project support unit, sustainable development goals (PSU-SDGs) now invites sealed bids from eligible bidders who wish to bid for projects.

1. Renovation of primary Health Centers,
2. Supply and distribution of wheel chairs
3. Supply and distribution of crutches.

Bidding will be conducted through the National Competitive Bidding as specified in the Bayelsa state procurement Laws and is open to all eligible bidders as enshrined in the law and manual.
NOTE: A bidder is not allowed to bid for more than three lots.
Bidders interested in the project(s) are to submit company’s profile which must contain the following:
i. Evidence of Registration with the Corporate Affairs Commission (CAC)
ii. Detailed company profile showing evidence of key personnel
ii. Tax Clearance Certificate for the immediate past 3 years
iv. Current Tax Clearance Certificate of at least 2 directors or proprietor(s) of the company for the past 3 years (i.e 2016,2017,2018)
v. Evidence of VAT registration and remittances
vi. Company’s audited financial account (indicating turnover) and funding information over the past 3 years by a firm of chartered accountants.
vii. Documentary Evidence of previous/ similar works in the past 3 years showing;
• Letter of award of contract
• Certificate of completion
viii. A sworn affidavit indicating that
• None of the directors of the company has been convicted in any court within and outside Nigeria for any criminal offence including fraud and financial impropriety.
• The company is not in receivership, insolvent or bankrupt.
ix. Any additional information that will enhance the potential of the contractor.
4.0 COLLECTION OF TENDER DOCUMENTS
Interested bidders are to collect bid documents from the project support unit of the sustainable development goals (PSU-SDGs), office of the special adviser to the Governor on SDGs 1st floor, Room 21, state Secretary Complex, Yenagoa, Bayelsa State from the date of publication of the advert between the hour of 9:00am and 3:00pm, Monday – Friday, excluding public holiday with evidence of payment of a non-refundable fee of n50, 000 (fifty thousand naira) only, per lot. The tender fee is to be paid into Sustainable Development Goals unity bank account Number 0037241326.
Please note that a copy of the receipt will be presented before collection of tender documents.
Bidding document requested by mail will be dispatched by registered/ speed post on payment of the postage cost. The project support unit, sustainable development goals will not be liable for postage delay (if any) in the delivery of the document or non-receipt of the same.
5.0 SUBMISSION OF TENDER DOCUMENT
All bids must be accompanied by bid security of one (1%) of the quoted contract sum.
Tender documents are to enclosed in two envelopes; one to contain “TECHNICAL BID” on the top of the right corner and the other to contain the “FINANCIAL BID” and boldly marked “FINANCIAL BID” on the top right corner. The two envelopes are to be placed in one large envelope which should be addressed to;

The number of lot(s) bided and the local Government Area should be written on the top hand right corner of the front envelop and also the name of the company should be written boldly on the other side of the envelope.
The envelopes are to be submitted in a tender box at the office of the project support unit, Room 21, state Secretariat Complex, Yenagoa, Bayelsa state, after they have been registered at the same office. Tenders are to be submitted not later than 12 noon on the 29th of May, 2019. Bid submitted after deadline shall be rejected.
All submission must be made in English Language.
6.0 BID OPENING
The bid opening will be immediately after the deadline of 29th of May, 2019 at 12 noon at the SSG’s Conference Hall, 1st floor, state Secretariat Complex, Bayelsa state.
Bidders or their representative as well as Civil Society Organizations will be invited to ensuring transparency and accountability to witness the public opening of the bids.
This advertisement “invitation to tender” shall be an award of contract to any intending contractor and all expenses incurred by any intending contractor is at her own expenses.
