The University College Hospital (UCH) Ibadan, a tertiary Hospital in Oyo State, South West Nigeria wishes to run her Mortuary on a PPP Arrangement.

The University College Hospital, Ibadan hereby requests for Proposal for the Privatisation of her Mortuary on a Public Private Partnership (PPP) arrangement from qualified individuals or firms with prior knowledge on PPP Interested applicants are required to meet the criteria stated below:

(3) Interested applicants are required to meet the criteria stated below:-
(i) A written documentation of project methods, comprehensive protect report and recommendation with timelines
(ii) A comprehensive description of all the Equipment needed to run such a Centre described above
(iii) A detailed description of human resource needs for such a Centre
(iv) A Comprehensive description of certifications needed to operate such a Centre.

Only qualified individual or firms with prior knowledge on a PPP Arrangement experience should submit proposal in response to this request. All proposals should include the following:
(a) A technical proposal that is not more than 15pages This technical proposal must provide an overview of the proposed project as well as the resume of personnel involved. In addition, the technical proposal should provide a proposed schedule and timeline as applicable
(b) Photocopy of certificate of incorporation with the Corporate Affairs Commission (CAC) of Nigeria
(c) Current Tax Clearance Certificate for years 2016, 2017 and 2018
(d) Three (3) years (unabridged) company audited Annual Report (2016, 2017 and 2018) certified by a Chartered Accountant or Auditor
(e) Evidence of Financial Capability/Reference letter from a reputable bank
(f) Evidence at current Certificate of Compliance from the industrial Training Fund (ITF) in compliance with the ITF Act, 2011, valid for year ending December 2019
(g) Evidence of current Certificate of Compliance from the Pension Commission (PENCOM) in compliance with the amended PENCOM Reform Act 2014, valid for year ending December 2019
(h) Verifiable list of previous/similar projects successfully executed in the past three (3) years supported with Letters of Award and Completion Certificates as applicable
(i) Registration on the National Database of Contractors, Consultants and Suppliers by inclusion of Interim Registration Report (IRR) Issued by the Bureau of Public Procurement (BPP) valid for year ending December 2019
(j) Current Sworn Affidavit disclosing:
(i) That the company/organization is not in receivership, insolvent or bankrupt
(ii) That the company/organization does not have any Director who has ever been convicted in any court in Nigeria or any other country for criminal offence(s) relation to fraud or financial impropriety.
(iii) Whether or not any officer of the relevant Committees of the University College Hospital, Ibadan or Bureau of Public Procurement was/is a former or present Director, shareholder or has any pecuniary interest in the bidding.
(iv) That all documents submitted are true and correct in particulars.
(k) Further innovative proposal that would enhance Nigerian content for the contract execution programme.
(l) Letter of Consent to allow tor verification of all the documents submitted including CAC, Inland Revenue, VAT remittances, Pension and Industrial Training Fund and company’s financial standing with the bankers

(i) Letter of Authorization from Original Equipment Manufacturers (OEM)
(ii) Catalogue of Equipment to be supplied from the Original Equipment Manufacturers (OEM).
(iii) In compliance with the executive order 003 of 2017 on support for Local content in Public Procurement, University College Hospital (UCH) Ibadan, is committed in providing opportunities for indigenous capacity development and utilization.
To ascertain prospective contractor commitment in the Nigerian content, the following documents must be submitted:
(1) Evidence of physical operational presence and set up in Nigeria
(2) Current organizational structure with evidence of the percentage of the management that are Nigerian nationals and the percentage of total workforce that are Nigerians.
(3) Nigerian content plan for the works goods and services to be provided using Nigerian resources.
(4) Number of new jobs to be created by this opportunity

Submission of Tender closes on 5th August, 2019 at 12:00noon. Any Tender received after this date and time will not be considered. The submitted documents will be opened on the same day 5th August, 2019 at 12:00noon at the Omolade Alade Hall of the School of Nursing, UCH, Ibadan, Bidders are required to be present at the opening.

Only Tenders that fulfill the above requirements end submitted within the stipulated dates shall be given consideration.
