INVITATION FOR TECHNICAL AND FINANCIAL BIDS FOR THE IMPLEMENTATION OF 2020 CAPITAL APPROPRIATION PROJECTS

1. INTRODUCTION
The Federal Medical Centre Owo was appropriated funds in the 2020 Capital Budget for the execution of its project. In compliance with Public Procurement Act 2007, the Federal Medical Centre Owo hereby invites all experienced competent contractors, suppliers, and manufacturers with relevant experience and good track records to tender for the execution of the following project.

3. ELIGIBILITY REQUIREMENTS
Interested companies are requested to submit the following documents/information with verifiable evidences for assessment/consideration;
(i) Evidence of Certificate of Incorporation with the Corporate Affaire Commission (CAC) including form CAC 2 and CAC 7 (or CAC 11)
(ii) Evidence of Company’s Income’s Tax Clearance Certificate for the last three (3) years (2017, 2018 and 2019) valid till 31st December, 2020;
(iii) Evidence of current Pension Compliance Certificate valid till 31st December, 2020;
(iv) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December, 2020;
(v) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2020;
(vi) Evidence of registration on the National database of Federal contractors consultants and service providers by submission of Interim Registration Report (IRR) expiring on 30/06/2020 or valid certificate issued by the Bureau of Public Procurement (BPP);
(vii) Evidence of financial capability by submission of reference letter from a reputable commercial bank in Nigeria, indicating willingness to provide credit facility for the project
(viii) Company’s Audited accounts for the last three (03) years (2017, 2018 and 20191 duly signed and stamped by a professional accounting audit firm;
(ix) Current Sworn Affidavit Certifying that:
– The company is not in receivership, Insolvent or bankrupt
– None of the directors has been convicted in any court of law or any criminal offence
– No offence of the relevant committees of the Federal Medical centre, Owo or the Bureau of Public Procurement is a former or present director, shareholder or has any peculiar interest in the company and that all information presented in the document are true and correct in all particulars.
(x) Of consent to allow for verification of Verifiable documentary evidence of at least three (03) similar jobs executed in the last five (05) years including letters of award; valuation certificate job completion certificates and photographs of the projects.
(xi) Letter all the documents submitted including CAC, Tax Clearance Certificate, Pencom, Industrial Training Fund (ITF), NSITF, BPP Interim Registration Report (IRR) and Company’s financial standing with the Banks.

4. COLLECTION OF TENDER DOCUMENTS
The tender documents are to be collected from the office of Head of Procurement Department on payment of a non-refundable Tender Fee of N10,000.00 per Lot into the FMC, Owo’s Remitta Account. The Hospital’s receipt and photocopy should be attached to the Tender Documents.

5. SUBMISSION OF DOCUMENTS
The tenderer shall enclose two hard copies each of the technical and financial bids, packaged separately in sealed envelope and clearly marked as “ Technical bid” and Financial bid” Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to:
The Medical Director,
Federal Medical Centre, Owo
The title of the project should be written at the top-left corner of the envelope and should not bear the name of the tendering contractor e.g. Supply of medical Equipment.

The completed and sealed tender documents should be deposited in the tenders Box in the office of the Medical Director FMC, Owo not later than 12:00 noon on Monday 4 of May, 2020.

6. OPENING OF TECHNICAL BIDS
The submitted Technical bids will be opened immediately after the deadline for submission at 12:00 noon on Monday 4th of May, 2020 at the Hospital’s Auditorium, Federal Medical Centre, Owo. Bidders/company representatives and interested members of the public including relevant professional bodies and Civil Societies are hereby invited to witness the opening of the Technical bids. The date of opening of financial bids will be communicated to pre – qualified bidders. Federal Medical Centre, Owo will not be held liable for misplaced or wrongly submitted bids.

GENERAL INFORMATION:
(a) Bids must be in English language signed by an official authorised by the bidder and courier delivery will not be considered.
(b) Bids submitted after the deadline for submission would be returned un-opened.
(c) The Federal Medical Centre, Owo is neither committed nor obliged to short-list any Contractor or to award the contract to any contractor or Agent.
(d) The FMC Owo reserves the right to reject any and/or all bidding packages.
(e) The FMC, Owo will only recognise and correspond with only authorized officers of the tendering companies and NOT through individuals or agents acting on their behalf.
(f) This advertisement for invitation shall neither be construed as a commitment on the part of the FMC, Owo, or shall it entitle any contractor to make any claim whatsoever and/or seek any indemnity from the FMC, Owo.
(g) FMC Owo also reserves the right to invoke the provision of section 28 of Public Procurement Act (PPA) without incurring any liabilities to the bidders.
(h) All costs will be borne by the bidders.
