DEADLINE: 29TH MARCH, 2021 FEDERAL NEUROPSYCHIATRIC HOSPITAL, ARO, ABEOKUTA P.M.B. 2002, ABEOKUTA, OGUN STATE EXTERNAL ADVERTISEMENT INVITATION TO TENDER FOR CONSULTANCY SERVICES FOR YEAR 2021 CAPITAL PROJECTS

(l) INTRODUCTION The Federal Neuropsychiatric Hospital, Aro, Abeokuta in implementing her Year 2021 Capital Projects hereby invites all interested, qualified and reputable Consultancy Firms with relevant experience and track record to tender for the following projects which would be funded by Year 2021 Capital Allocation:

DESCRIPTION SCOPE LOT 2021 A Construction of 250 -Seater Auditorium for the School of Psychiatric Nursing and other Health Professionals at Aro Annexe Ø ArchitecturalØ Structural EngineeringØ Quantity SurveyingØ Mechanical and Electrical LOT 2021 B Construction and Equipping of Purpose- Built Laboratory Building at Aro Annexe Ø ArchitecturalØ Structural EngineeringØ Quantity SurveyingØ Mechanical and Electrical LOT 2021 C Construction of a Multi-Purpose Built Psychology Building at Aro Annexe Ø ArchitecturalØ Structural EngineeringØ Quantity SurveyingØ Mechanical and Electrical LOT 2021 D Rehabilitation and Equipping of Laundry Building at Aro Annexe Ø ArchitecturalØ Structural EngineeringØ Quantity SurveyingØ Mechanical and Electrical LOT 2021 E Drilling of four (4) Boreholes and Reticulation of Water System at Aro and Lantoro Annexe Ø Geophysist LOT 2021 F Procurement of 2.5MVA Transformer for 33/11KVA line at Aro Annexe Ø Mechanical and Electrical LOT 2021 G Expansion and Equipping of Theatre Room at General Medical Practice Clinic (GMPC) at Aro Annexe Ø ArchitecturalØ Structural EngineeringØ Quantity SurveyingØ Mechanical and Electrical LOT 2021 I Construction of two (2) Additional Wards at Aro Annexe Ø ArchitecturalØ Structural EngineeringØ Quantity SurveyingØ Mechanical and Electrical

(2) ELIGIBILITY CRITERIA Interested Consultants are required to submit their Firm’s documents in sequential order as listed below:

(a) Firm’s Certificate of Incorporation with Corporate Affairs Commission (CAC) including forms CAC 1.1 or CAC 2, and CAC 7, Business Name is also acceptable;

(b) Evidence of Firm’s Tax Clearance Certificate (or Personal income Tax clearance Certificate of all Partners in case of Business Name) for the last three (3) years (i.e. 2018, 2019 and 2020) valid till 31 December, 2021;

(c) Evidence of Firm’s registration with relevant regulatory Professional Body(ies) e.g. COREN/QSRBN/ARCON/CORBON/ SURCON etc.;

(d) Evidence of Pension Clearance Certificate valid till 31st December, 2021;

(e) Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31st December, 2021;

( f) Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 3st1 December, 2021;

(g) Evidence of Registration on the Bureau of Public Procurement (BPP) National Database of Contractors, Consultants and Service Providers by inclusion of Interim Registration Report (IRR) valid till 31 December, 2021 or valid Certificate issued by BPP;

(h) Sworn Affidavit disclosing the following:

None of the Directors of the Firm has ever been convicted by any Court of Law; No officer of the relevant Committees of Federal Neuropsychiatric Hospital, Aro, Abeokuta or the Bureau of Public Procurement is a former or present Director, Shareholder or has any pecuniary interest in the Bidder and to confirm that all information presented in its bid are true and correct in all particulars.

(i) Verifiable documentary evidence of at least three (3) similar jobs successfully executed in the last five (5) years supported with letters of award, Activity Description, Client, Location, Value, Status (ongoing or completed) and photographs of the project etc.;

(j) Firm’s Audited Accounts for the last three (3) years (i.e. 2018, 2019 and 2020);

(k) Firm’s Profile with the Curriculum Vitae of key staff to be deployed for the project, including copies of their Academic/Professional qualifications (e.g. COREN, QSRBN, ARCON, CORBON etc.);

(l) All documents for submission must be transmitted with a covering/forwarding letter under the Firm’s Letterhead bearing amongst others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM Number) and e-mail address. The Letterhead must bear the Names and Nationalities of the Directors of the firm at the bottom of the page duly signed by the Authorised officer of the Firm.

Note: Documents (a) – (1) are RESPONSIVE CRITERIA, the absence of any automatically disqualifies the Bidder.

(3) SUBMISSION OF EOI DOCUMENTS Interested firms are to submit two (2) bound of Tender Documents separated by dividers and arranged as indicated above. The documents should be submitted in a sealed envelope and addressed to

and clearly marked with the name of the project and Lot Number.

Furthermore, the reverse of each sealed envelope should bear the name and address of the Bidder and dropped in the designated Bid Box located at the Office of the Provost and Medical Director not later than 12:00 noon on Monday, 29 March 2021.

(4) OPENING OF EOI DOCUMENTS The EOI will be opened immediately after the deadline for submission 12:00 noon on Monday, 29 March 2021 in the Boardroom, Postgraduate Building, Federal Neuropsychiatric Hospital, Aro, Abeokuta. Please, ensure you sign the Bid submission Register in the Office of the Head of Procurement, New Administrative Building, 1 st Floor, Room 06, Federal Neuropsychiatric Hospital, Aro, Abeokuta as the Hospital will not be held liable for misplaced or wrongly submitted tender. (5) GENERAL INFORMATION (a) The EOI must be in English Language and signed by an official authorised by the Bidder;

(b) Any EOI submitted after the deadline for submission will be returned un-opened;

(c) Bidders should not bid for more than two (2) Lots;

(d) All costs will be borne by the Bidders;

( e) Presence of bidders/representatives and members of the public shall not be required during the bid opening ceremony in compliance with COVID-19 Guidelines. The Bids Opening will be covered by video recording with the presence of two (2) representatives of private sector professional bodies and Civil Society Organisation in the areas of Anti- Corruption/Transparency;

( f) Bidders can submit through Post (Courier Services)

( g) The Federal Neuropsychiatric Hospital, Aro, Abeokuta is not bound to shortlist any Bidder and reserves the right to annul the Procurement Process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
