INVITATION TO TENDER FOR THE CONSTRUCTION OF A NEW CENTRAL REFERENCE HOSPITAL OKENE, THE RENOVATION / REMODELLING OF THE EXISTING ZONAL HOSPITAL OKENE, AND PROCUREMENT OF MEDICAL LABORATORY AND OFFICE EQUIPMENT / FURNITURE FOR THE HOSPITAL
Tenders are hereby invited from competent Contractors for the Remodeling of the Zonal Hospital Okene as listed below:
| LOT 1 | Construction of the New Hospital Complex. |
| LOT 2 | Construction of CT and MRI Block |
| LOT 3 | Construction of Perimeter Fencing of the Hospital |
| LOT 4 | Construction of Oxygen Plant |
| LOT 5 | Renovation / Remodeling of the old Hospital |
| LOT 6 | Procurement and Installation of Medical / Clinical / surgical /Laboratory Equipment |
| LOT 7 | Procurement and Installation of Electrical Gadgets for the Hospital |
| LOT 8 | Procurement and Installation of Hospital Equipment and Furniture |
| LOT 9 | Procurement and Installation of Office furniture |
| LOT 10 | Procurement and Installation of Radiology / Imaging Equipment |
| LOT 11 | Renovation and Remodeling of MCH Agassa. |
REQUIREMENTS
(1) Evidence of Registration with CAC
(2) Tax Clearance certificate for past three years
(3) Audited Account for the past three years
(4) Evidence of Registration with Kogi State Bureau of Public Procurement.
(5) Evidence of VAT Registration and Remittance
(6) Evidence of Financial Capability from a reputable Bank in Nigeria
(7) Evidence of similar job done in the last three years
(8) Comprehensive profile of the Contractor with list of key Personnel and Equipment.
(9) Evidence of payment of non-refundable Contract fee.
COLLECTION OF TENDER DOCUMENTS;
Tender documents are available from the Department of Health Planning. Research and Statistics, Ministry of Health, Lokoja on payment of the non-refundable fee into a specified Kogi State Board of Internal Revenue Bank Account.
SUBMISSION AND OPENING OF BIDS DOCUMENTS:
The duly completed bid documents should be forwarded to the Hon. Commissioner, Ministry of Health, PMB 1068, Lokoja;
Attention; Director Health Planning, Research and Statistics in a sealed envelope marked MOH/MTB/01/2020 with the Lot clearly written on the top left hand corner of the envelope to reach him not later than 12.00noon 19, March 2020 at which time bids, will be opened in the presence of bidders or their accredited representatives.
NOTE;
(i) The Ministerial Tenders; Board reserves the right to reject any or all bids.
(ii) This invitation should not be construed as a commitment on the part of the Ministerial Tenders Board to enter into any form of contract with any Contractor or into communication with any Contractor nor shall it entitle any Contractor to make claims from the Ministerial Tenders Board.
