INVITATION FOR PRE-QUALIFICATION AND TENDER OF CONTRACTORS FOR THE SUPPLY OF OFFICE EQUIPMENT AND TEACHING AIDS
The Nurse Tutors Training Programming (Federal Ministry of Health) Kaduna invites qualify and reputable contractors with relevant experience and track record for pre-qualification and tender for both Technical and Financial for the followings.
SCOPE OF SUPPLY
LOT 1. Furnishing of the Extension of students Block (Phase II)
(1.0) PRE-QUALIFICATION ELIGIBILITY REQUIREMENTS:
In other to be eligible for pre-qualification, invited contractors must submit the followings in the company’s profile:
(a) Evidence of Certificate of incorporation by the Corporate Affairs Commission (CAC) including form CAC2 and CAC7.
(b) Evidence of current company income tax clearance certificate from FIRS to December 31st, 2021.
(c) Evidence of VAT Registration and Remittance’s Certificate
(d) Evidence of Current pension clearance certificate (PENCOM) valid till 31st December, 2021.
(e) Evidence of Industrial training fund (ITF) compliance certificate valid till December 31st, 2021.
(f) Evidence of Nigerian Social Insurance Trust Fund (NSITF) clearance certificate valid till December 31st 2021.
(g) Evidence of Current Registration on the National Data Base of Federal Contracts, Consultants and Service Providers by Submission of (BPP) Interim Registration Report (IRR) expiring 31st December, 2021.
(h) Sworn Affidavit disclosing whether or not any officer of the Nurse Tutors Training Programme is a former Director, Share Holder or has any pecuniary interest in the bidder and to confirm that all information presented in its bids are true and correct in all particulars.
(i) Company’s audited account for the last 3years 2019, 2020, 2021.
(j) Documentary company’s profile and activities.
(k) A tender fee of N50, 000 per LOT applies
(l) All documents for submission must be transmitted with a covering letter under the company letter head paper show in addresses, e-mail and telephone numbers.
(2.0) SUBMISSION OF DOCUMENTS
Prospective Bidders are to submit the both pre-qualification and financial tender in a separate envelope marking pre-qualification documents and financial tender on each envelope. The two envelopes are to be enveloped in a one larger envelope and addressed to the coordinator Nurse Tutors Training Programme Kaduna and deposited at designated tender box at Nurse Tutor Training office building Kaduna Polytechnic, Tudun Wada Kaduna on or before 12:00 noon, 7 Dec, 2022.
OPENING OF PRE-QUALIFICATION DOCUMENTS
The technical bid will be opened immediately after the deadline of submission at 12 noon on 7 Dec, 2022 in the Nurse Tutors Training Program Office.
(a) Bids must be in English Language and signed by an official authorized by the bidder.
(b) Bids submitted after the deadline for submission will be returned unopened
(c) All cost to be borne by the bidders.
