Jigawa State Government- Invitation To Pre-qualification And Tender In Respect Of Supply And Installations Of Medical Equipment To Newly Upgraded Primary Health Centres To General Hospitals Across The State (2 Lots)

INVITATION TO PRE-QUALIFICATION AND TENDER IN RESPECT OF SUPPLY AND INSTALLATIONS OF MEDICAL EQUIPMENT TO NEWLY UPGRADED PRIMARY HEALTH CENTRES TO GENERAL HOSPITALS ACROSS THE STATE
PROCUREMENT NO: MOH/SEC/1.5/611/I/151

(a) PREAMBLE
The Government of Jigawa State in its effort to increase efficient and effective health care service delivery in the State, hereby invites tenders from reputable contractors for the above named projects

(A) SCOPE OF WORK
The projects are divided into lots 1, &2. as per details below: –

(B) ELIGIBILITY REQUIREMENTS

Bidding document should be submitted along with following documents:

(i) Certificate of incorporation with Corporate Affairs Commission

(ii) Certified True Copy of Memorandum and Article of Association

(iii) Evidence of registration with Jigawa State Due Process and Project Monitoring Bureau

(iv) Tax clearance certificate for the last three (3) years

(v) Audited Account for the Last One (1) Year

(vi) VAT Registration and Evidence of Past Remittance

(vii) Evidence of financial capability and banking support

(viii) Company’s Average Annual Turn Over of not less than N100 Million

(ix) That the Company has fulfilled all obligations to pay taxes, pensions, ITF Contribution and social security contributions

(x) Evidence of execution and completion of similar Projects at least 2 in the last 5 years

(xi) Verifiable evidence of Company profile and Technical Qualification of key personnel registered with relevant professional bodies (COREN CORBON, QSRBN, ARCON etc.

(xii) A sworn affidavit certifying as Follows;

(a) That the document submitted for the prequalification exercise are not only genuine but correct

(b) That none of the Directors of the Company has been convicted by any court of law

(c) That the Company is not bankrupt.

(d) That none of the officers of Jigawa State/Local Government is a present or former director of the Company.

(xiii) Any other relevant information that will help in determining the Company’s suitability for the work

(C) COLLECTION AND SUBMISSION OF TENDER DOCUMENTS
Bidding documents could be obtained through Jigawa State e-procurement portal (https://jigawa-eproc.eurodyn.com/epps/home.do upon payment of non-refundable fee of Five Hundred Thousand Naira only (N500,000) only to the state Ministry of health from Friday 2 December, 2022 to Friday 13 January, 2023 by 12:00 noon from 9.00 am to 5 00 pm daily.

(D) SUBMISSION OF TENDER DOCUMENT
(i)  Company are to submit their bids on Jigawa state e-procurement portal ((https://jigawa-eproc.eurodyn.com/epps/home.do) on or before 13 January, 2023 by 12.00 Noon.

(ii) The Opening date is Friday 13/01/2023 By 12:00pm

(iii) A scanned copy of bid security of not less than 2% of the bid price either in form of insurance bond or bank guarantee addressed to Honorable Commissioner, Ministry of Health Jigawa State, Dutse, Jigawa State should also be uploaded into the portal.

(iv) Pre-qualification and financial bids should be submitted in two different sealed envelopes, (1 original and 2 photocopies in a bigger envelope) addressed to the Honorable Commissioner Ministry of Health, block B New Secretariat Complex, Takur Dutse.

(v) Similarly, the original of company’s profile, signed forwarding letter with attached bill, bid security and a copy of the sworn Affidavit in hard copy should be send to Honorable Commissioner. Ministry of Health Jigawa State, Dutse, Jigawa State

(vi) Bids shall be valid for a period of 120 days (4month) after Bid opening.

(vii) Bids submitted after the deadline for submission will be returned un-opened.

(F) GENERAL INFORMATION
However, bidders should note the following information

(1) Bids submitted after deadline for submission will be denied access to the portal.

(2) Only financial bids of successful pre-qualified bidders will be opened.

(3) This advert shall not be construed as not be construed as commitment on it entail a Company to make any claim(s) whatsoever Ministry of Health by Virtue of such Company having responded to this advertisement.

(4) Bidders with the same subsidiary relationship are not allowed to partake in the same lot.

(5) All cost will be borne by the bidders as the Jigawa state Ministry of Health will not be held responsible for any cost or expenses incurred by any interested party i.e in connection with any response to this invitation.

(6) The Jigawa State Ministry of Health reserves to annul the procurement process at any time without incurring any liabilities.
