REQUEST FOR QUOTATION FOR THE PURCHASE OF HOSPITAL EQUIPMENT

Description

  • Women in the New Nigeria and Youth Empowerment Initiative (WINN) is in Partnership with Christian Blind Mission (CBM) International with funding from GFFO working in Jere, Konduga, MMC, and Gwoza LGAs, Borno State.
  • WINN is seeking qualified suppliers to provide a price quote for hospital equipment as seen in the table below. Please refer to the Term and Conditions (on last Page) in preparing your quotation.

Additional Terms & Conditions

  • Delivery of Sample: The preferred supplier will be requested to provide a sample of hospital equipment before signing the contract with WINN within 1-3 days of notification.
  • The detailed financial quote should be in Naira.
  • The quotation should state clearly the validity period of the quote (which should not be less than a year), payment terms, and availability of the product.
  • Applicable withholding tax will be deducted from price quotes and remitted to appropriate statutory bodies.
  • Liquidated Damage: All aspects of safe delivery to WINN shall be the exclusive responsibility of the supplier. If the supplier fails to deliver the goods on or before the stipulated date, however, if he or she fails to supply within the stipulated timeframe, his or her contract can be revoked  and given to another vendor or supplier.
  • Risk Purchase. If a successful vendor fails to supply hospital equipment by the stipulated date, WINN reserves the right to terminate the contract.
  • Marking: Each package shall be marked and labelled in accordance with the requirements of the Standard Organization of Nigeria (SON).

Packing:

  • Loose supplies, damaged packing, or tampered or damaged-labelled supplies shall not be accepted under any circumstances.
  • All primary packing containers should strictly conform to the specification included in the relevant package.
  • Packing should be done to prevent damage or deterioration during transit.

Quotations and Submission Requirements

The quotation shall contain the following documents:

  • Soft or hard copies of the company profile (including legal, identity, trade license, CAC, tax clearance certificate, proof of similar contracts executed, other relevant legal documents, and license of dealership if applicable)
  • Copies of FIRS printouts of Tax Identification Number (TIN)
  • Detailed description of the quoted items (with pictures if possible) as listed in the requirements list.
  • Provide a list of a minimum of 3 relevant references for similar supplies for the last two years, preferably from NGOs supported by evidence of supplies or delivery of items.

Evaluation Criteria

  • Quotes will be evaluated based on their responsiveness to the specifications, financial quote, validity of quote, evidence of past or current delivery of the required services, qualification (experience), after-sale service, and agreement to WINN payment terms and delivery time.

Method of Application
Interested and qualified candidates (suppliers) should submit their quotations (hard copy) on their company’s letterhead paper, duly signed and stamped by an authorized person of the company, in a sealed envelope addressed to the WINN Procurement Committee. Hard copies must be delivered at House No. F17, Tujjani Maina Street, NUJ Bama Road, Maiduguri, BornoState.

Alternatively, Send documents electronically to quotations@winnigeria.org with the subject “Quotation for the Purchase of Hospital Equipment. “All documents should be scanned into one file.

Note
A quotation (hard copy and soft copy) must reach WINN on or before the closing date and time to have a chance of being considered as the preferred supplier.

  • Incomplete Quotations shall not be examined.
  • WINN reserves the right to reject any and all bids not considered to be in the best interest of the organizations and/or people it serves.
