INVITATION FOR EXPRESSION OF INTEREST (EOI) AND TENDER FOR THE EXECUTION OF YEAR 2025 CAPITAL PROJECTS
1.0 INTRODUCTION
1.1 The Federal Teaching Hospital, Lokoja hereby invites reputable and interested Contractor with relevant experience and good records of excellent accomplishment of performance to submit Expression of interest for the appointment as Project Consultant and Tender documents for the execution of the year 2025 Capital projects.
2.0 SCOPE OF WORKS/SUPPLY AND SERVICE
2.1 CATEGORY A: CONSULTANCY SERVICE:
LOT 1 (Project Code ocds-gyl66f-521028029-000037): Consultancy Service for Construction & Renovation of Physiotherapy, Paediatrics, medical/Surgical, Orthopaedic, Male Medical, Mortuary/Histopathology, Isolation Building and Due Processes etc., for 2025 Projects;
LOT 2 (Project Code ocds-gyl66f-521028029-000043): Medical Outreach on Malaria, Typhoid, Diabetic and other related Medicals;
LOT 3 (Project Code ocds-gyl66f-521028029-000045): Medical Outreach in Kabba-Bunu/Ijumu Federal Constituency, Kogi State;
2.2 CATEGORY B: WORKS:
LOT 1 (Project Code ocds-gyl66f-521028029-000026): Renovation/Extension of Male Medical, Pediatric Surgical Wards and Installation of Solar Power Backup
LOT 2 (Project Code ocds-gyl66f-521028029-000028): Construction of Medical Isolation Building
LOT 3 (Project Code ocds-gyl66f-521028029-000034): Construction, Procurement and Installation of Fibre Network Infrastructure & Additional Computer Desktop for Existing Electronic Medical Records (EMR) System
LOT 4 (Project Code ocds-gyl66f-521028029-000036): Construction of Mobile Office Cabin & Furnishing
LOT 5 (Project Code ocds-gyl66f-521028029-000042): Renovation of Selected Primary health Centres in Okengwe Ehi, Kogi State
LOT 6 Completion of Oxygen Plant & Distribution to Wards/Theatres, Etc.
3.0 ELIGIBILITY REQUIREMENTS
Interested consultants/contractors must submit two (2) bound EoI or Tender documents duly signed, paginated, separated by dividers and arranged in the order outlined hereunder:-
a. Colour copy of Evidence of Certificate of Incorporation issued by the Corporate Affairs Commission (CAC) including Form CAC1.1 or CAC2 and CAC7.
b. Colour copy of Evidence of Company’s Income Tax Clearance Certificate for the last three (5) years (2022, 2023 and 2024) valid till 31st December, 2025 with a minimum average annual turnover of N300m for category B: LOT 1, LOT 2 & LOT 6;
c. Colour copy of Evidence of Pension Clearance Certificate valid till 31stDecember, 2025 (this requirement is only applicable to bidders whose number of staff is 15 and above);
d. Colour copy of Evidence of Industrial Training Fund (ITF) Compliance Certificate valid till 31stDecember, 2025 (this requirement is only applicable to bidders whose number of staff is 5 and above or the bidder’s annual turnover is N50m and above);
e. Colour copy of Evidence of Nigeria Social Insurance Trust Fund (NSITF) Clearance Certificate valid till 31st December, 2025;
f. Evidence of current License/Accreditation with Computer Professional Registration Council of Nigeria and National Information Technology Development Agency (NITDA) for Category B: LOT 3;
g. Colour copy of Evidence of Registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31stDecember, 2025 or valid Certificate issued by BPP.
h. Company’s Audited Accounts for the last three (3) years 2022, 2023 & 2024;
i. Reference Letter from a reputable Commercial Bank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed;
j. Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including Letters of Awards, Valuation Certificates, Job Completion Certificates and photocopy of the projects;
k. Sworn Affidavit disclosing whether or not any officer of the relevant committees of Federal Teaching Hospital, Lokoja or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
l. For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided with Sworn Affidavit;
m. All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing among others, the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No.), and e-mail address. The Letterhead Paper must bear the Names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.
n. Evidence of Firm’s registration with relevant professional body MDCN, for Category A: LOT 2 & LOT 3;
o. Company’s Profile with the Curriculum Vitae of Key Staff to be deployed for the project, including copies of their Academic/Professional qualifications such as COREN, QSRBN, ARCON, CORBON, ICAN, ANAN etc.;
4.0 SUBMISSION OF EOI AND TENDER DOCUMENTS
Prospective bidders/firms are to submit two (2) hard copies (original & photocopy) of the tender/EoI documents for the Lot desired, sealed in an envelope. Thereafter, put the two sealed envelopes together in a bigger sealed envelope addressed to;

and clearly marked with (Category, LOT/No. and project title). Furthermore, the reverse of each sealed envelope should bear the name and address of the bidder and drop it in the Tender Box at the entrance of the Board room, located in the Annex Administrative block of Federal teaching Hospital, Lokoja from Monday to Friday during official working hours (8am-3:59pm) and not later than 12:00 noon on 14th April, 2025. Note that the Federal Teaching Hospital, Lokoja, will not be held liable for misplaced or wrongly submitted bid.
5.0 OPENING OF TECHNICAL BIDS
The EOI/Tender documents will be opened immediately after the deadline for submission at the Boardroom of the Annex Administrative Block at 12:00 noon 14th April, 2025 in Federal Teaching Hospital, No. 1 Salihu Ibrahim Way Lokoja, Kogi State in the presence of bidders or their representatives. Please, ensure that you sign the Bid Submission Register in the office of procurement unit at the Annex Administrative block. Presence of bidders/representatives are welcome during the bid opening exercise in compliance with the public procurement Act, 2007 and the presence of Civil Society Organization registered with BPP for such purpose:
6.0 GENERAL INFORMATION
a. Bids must be in English Language and signed by an official authorized by the bidder;
b. Bids submitted after the deadline for submission would be returned un-opened;
c. No company or related company should bid for more than two (2) Lots;
d. All costs will be borne by the bidders;
e. Tender documents can also be submitted through Post (Courier Services);
f. Only pre-qualified bidders at technical evaluation will be invited at a later date for financial bids opening;
g. The Federal teaching Hospital, Lokoja is not bound to pre-quality/shortlist any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
