REQUEST FOR EXPRESSION OF INTEREST (EOI) FROM SERVICE PROVIDERS FOR LANDSCAPING OF THE FEDERAL TEACHING HOSPITAL KATSINA
1.0 INTRODUCTION
The Federal Teaching Hospital Katsina hereby requests for Expressions of Interest (EOIs) for prequalification from qualified and reputable service providers with proven experience in design and supervision of project implementation for Landscaping of the Federal Teaching Hospital Katsina.
2.0 SCOPE OF SERVICE
- Landscape design of the Federal Teaching Hospital Katsina. Supervision of project implementation.
3.0 ELIGIBILITY REQUIREMENTS:
Firms that wish to respond to this request for Expression of Interest should submit two (2) bound EOI documents duly signed, paginated, separated by dividers and arranged in the outlined hereunder:
i. Evidence of certificate of incorporation with the Corporate Affairs Commission (CAC) including form CAC2 and CAC7;
ii. Evidence of company Income Tax Clearance Certificate for the last three years; (2022, 2023, 2024) valid till 31st December 2025;
iii. Evidence of current Pension Compliance Certificate valid till 31st December 2025;
iv. Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December 2025;
v. Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December 2025;
vi. Evidence of registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 31st December 2025 or valid Certificate issued by BPP;
vii. Evidence of firm registration with relevant professional bodies COREN/QSRBN/ARCON/CORBON/SURCON etc.
viii. Sworn affidavit disclosing whether or not any officer of the relevant committees of the Federal Teaching Hospital, Katsina or the Bureau of Public Procurement is a former or present director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
ix. Company’s Audited Accounts for the last three years (2022, 2023, 2024);
x. Evidence of Financial capability to execute the project including reference letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed;
xi. Company Profile with the curriculum vitae of Key Staff to be deployed for the project including copies of their academic/professional qualifications.
xii. Verifiable documentary evidence of at least three (3) projects with similarity to the proposed project executed in the last five (5) years. The documentary evidence should contain letters of awards, job completion certificates, photographs of the projects and the current status of the projects. The projects executed should include establishment of Site visits and technical assessment and supervision of project implementation in Nigeria.
4.0 SUBMISSION OF EOI DOCUMENT:
All documents for submission must be submitted with a covering/forwarding letter under the Company’s letter headed paper bearing amongst others, the registration number (RC) as issued by Corporate Affairs Commission (CAC), Contact Address, Telephone Number (Preferably GSM No.) and e-mail address. The letter headed paper must bear the names and nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm;
5.0 OPENING OF EOI DOCUMENTS:
Interested firms are to submit two (2) bound of expression of interest (EOI) documents separated by dividers and arranged as indicated above. The documents submitted in sealed envelopes and addressed to the Chief Medical Director, Federal Teaching Hospital Katsina, Murtala Mohammed Way, P.M.B 2121, Katsina. The sealed envelope should be marked with the name of the project and Lot number. The reverse side of each sealed envelope should have the name and address of the bidder and dropped in the designated tender box in the office of the Medical Director not later than 12noon, 9th June 2025.
The EOI documents will be opened immediately after the deadline for submission by 12noon 9th June, 2025 in the conference hall of the Federal Teaching Hospital, Katsina in the presence of bidders or their representatives. Prospective bidders should ensure they sign the bid submission register in the office of the Chief Medical Director, Federal Teaching Hospital, Katsina as the Federal Teaching Hospital, Katsina will not be held liable for misplaced or wrongly submitted bid. Please note:
a. EOI must be in English Language and signed by an official authorized by the bidder;
b. EOI submitted after the deadline for submission would be returned unopened;
c. Bidders should not bid for more than two (2) Lots;
d. All costs will be borne by the bidders;
e. All shortlisted bidders will be contacted at a later date for collection of tender document;
f. The Federal Teaching Hospital, Katsina is not bound to shortlist any bidder and reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act 2007.
