INVITATION TO TENDER FOR THE REVITALIZATION AT PRIMARY HEALTH CARE CENTRE (COMPREHENSIVE HEALTH CENTRE) IGBOATA, ILORO IJERO LGA, PRIMARY HEALTH CARE CENTRE (COMPREHENSIVE HEALTH CENTRE), ODE-1 GBONYIN LGA AND PRIMARY HEALTH CARE CENTRE (COMPREHENSIVE HEALTH CENTRE) OKE EMURE, EMURE LGA.
1.0 INTRODUCTION
1.1 The Ekiti State Primary Health Care Development Agency has been given approval by the National Primary Health Care Development Agency to commence the Implementation of the revitalization of three (3) Primary Health Care Facilities in Ekiti State to improve the quality of service delivery and Human Capital Development under the Basic Health Care Provision Fund Program (BHCPF).
1.2. Further to this and in compliance with the Ekiti State Public Procurement (Re-Enactment) law No.4 of 2020 i hereby invites interested registered, competent and reputable contractors to tender bids for the Revitalization at Primary Health Care Centre (CHC) Igboata, Iloro at Ijero LGA, Primary Health Care Centre (CHC) Ode-1 at Gbonyin LGA and Primary Health Care Centre (CHC), Oke Emure at Emure LGA.
2.0. PROJECTS
PROJECT I: Revitalization at Primary Health Care Centre (Comprehensive Health Centre) Igboata, Iloro at Ijero LGA
PROJECT II: Revitalization at Primary Health Care Centre (Comprehensive Health Centre) Ode-1 at Gbonyin LGA.
PROJECT III: Revitalization at Primary Health Care Centre (Comprehensive Health Centre) Oke Emure, at Emure LGA.
3.0 SCOPE OF WORKS (RENOVATION WORKS)
3.1 Alteration/Demolition works
3.2 Frame work
3.3 Roof work
3.4 Windows work
3.5 Doors
3.6 Electrical Installation
3.7 Mechanical installation
3.8 Floor, wall and Ceiling finishing
3.9 Painting and Decoration
3.10 External works (scope of works covers all other renovation items).
4.0 SCOPE OF WORKS (WATER SUPPLY AND SANITATION)
4.1 Drilling of boreholes
4.2 Solar-powered pump
4.3 Overhead tank
4.4 Reticulation to the facilities.
5.0 ELIGIBILITY REQUIREMENTS FOR TENDER
To be eligible, the prospective contractors must possess the following:
1. Evidence of Certificate of Incorporation with Corporate Affairs Commission (CAC) including forms CAC 02 and 07
2. Evidence of Certificate of registration with Ekiti State Bureau of Public Procurement (BPP) for year 2025
3. Evidence of payment of Development levy for three years 2022, 2023 & 2024
4. Evidence of Company Income Tax Clearance Certificate for three years 2022,2023 & 2024
5. Company’s Audited Accounts for three years (2022, 2023, and 2024)
6. Evidence of relevance previous work/ projects satisfactorily executed and this include: Award letters and Completion Certificates as executing the projects
7. Company Profile with the Curriculum Vitae of key staff to be deployed for the project including copies of the Academic/Professional Qualifications
8. Evidence of financial capability to execute the project including Reference Letter from a Reputable Commercial Bank and willingness to provide credit facilities for the execution of the project when needed
9. Evidence of Registration of Business Premises in Ekiti State for the 2025
10. VAT Registration Certificate
11. Each would-be bidder is expected to pay a non-refundable tender fee of N50,000
12. All documents for submission must be transmitted with a covering/forwarding letter under the Company/Firm Letter Head paper.
5.0 COLLECTION OF TENDER DOCUMENTS
Interested companies are to collect relevant documents from the State Primary Health Care Development Agency on the payment of a non-refundable tender fee as may be indicated on the notice Board of the Agency
6.0 SUBMISSION OF TENDER DOCUMENTS
Prospective bidders are to submit both technical and financial bids for the project to the Procurement Department of the Agency and addressed to the Executive Secretary, State Primary Health Care Development Agency, Ado-Ekiti not later than 21 days after the publication.
All valid tender documents should be neatly packed and submitted in sealed envelope marked TENDER FOR THE REVATILIZATION OF HEALTH FACILITIES IN EKITI STATE and addressed to the Secretary, Tenders Board, Ekiti State Primary Health Care Development Agency, Ado-Ekiti.
7.0 OPENING OF BIDS

8.0 GENERAL INFORMATION
8.1 Bids must be in English Language and signed off by an official authorized by the bidder;
8.2 Prospective bidders are invited to note that in line with the affirmative policy of the Government of Ekiti State, qualified Women-owned and Women-led vendors shall be strongly considered.
8.3 You are advised to visit the site for the purpose of inspection and reasonable pricing of the Bills of Quantities.
8.4 All costs will be borne by the bidders;
8.5 The State Primary Health Care Development Agency reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Ekiti State Public Procurement (Re-enactment) Law No.4 of 2020
