FEDERAL MEDICAL CENTRE (FMC) KATSINA MURTALA MUHAMMAD WAY JIBIA BYPASS P.M.B 2121, KATSINA INVITATION TO TENDER FOR 2020 CAPITAL PROJECTS

1. INTRODUCTION: (1.1) In compliance with the requirements of the Public Procurement Act 2007, the Federal Medical Centre Katsina hereby invites reputable companies/contractors with proven experience to undertake the under listed 2020 Capital Projects.

2. SCOPE OF WORKS/SUPPLY

2.1 GOODS:

Lot 1 Procurement, Supply and Installation of Ophthalmic Equipment Lot 2 Procurement Supply and Installation of Dental Equipment

2.2 WORKS:

Lot 3 Construction of Power house/Panel room Lot 4 Construction of ENT Complex Lot 5 Remodeling/Expansion of Emergency Pediatric Unit Lot 6 Remodeling/Expansion of Accident and Emergency Complex Lot 7 Construction of Male Surgical Ward Lot 8 Construction of Female Surgical Ward Lot 9 Construction of 6-suite Modular Theatre

3. ELIGIBILITY REQUIREMENTS (i) Evidence Of certificate of incorporation with the Corporate Affairs Commission (CAC) including form CAC2 and CAC7; (ii) Evidence of company Income Tax Clearance Certificate for the last three years Expiring on 31st December 2019. (iii) Evidence of current Pension Compliance Certificate valid till 31st December 2019: (iv) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December 2010; (v) Evidence of current Nigeria Social Insurance Trust Fund (INSITF) Compliance Certificate valid till 31st December 2010; (vi) Evidence of registration on the National Database of Federal Contractors, Consultants and Service Providers by submission of interim Registration Report (IRR) expiring on 31st January, 2020 or valid Certificate issued by BPP; (vii) Sworn affidavit disclosing whether or not any officer of the relevant committees of the Federal Medical Centre Katsina or the Bureau of Public Procurement is a former or present director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars; (viii) Company’s Audited Accounts for the last three years (2017, 2018, 2019); (ix) Evidence of Financial capability to execute the project by submission of reference letter from a reputable commercial Dank in Nigeria, indicating willingness to provide credit facility for the execution of the project when needed; (x) Company’s Profile with the curriculum vitae of Key Staff to be deployed for the project, including copies of their academic/professional qualifications (e.g. COREN, QSRBN, ARCON, CORBON, etc.); (xi) For Lots 1 & 2, Letter of Authorization from the original manufacturers listed in the Approved policy for procurement of Health and Medical Equipment for Tertiary Hospitals in Nigeria for bidders of Lots published by the BPP/FMOH/SON/NNRA, year 2017-2018 (2nd Revision); (xii) Verifiable documentary evidence of at least three (3) similar jobs executed in the last five (5) years including letters of awards, valuation certificates, job completion certificates and photographs of the projects; (xiii) All documents for submission must be transmitted with a covering/forwarding letter under the Company’s letter headed paper bearing, amongst others, the registration number (RC) as issued by Corporate Affairs Commission (CAC), Contact Address, Telephone Number (Preferably GSM No.) and e-mail address. The letter headed paper must bear the names and nationalities of the Directors of the company at the bottom of the page, duly signed by the authorized officer of the firm;

4. COLLECTION OF TENDER DOCUMENT Interested companies are to collect the Standard Bidding Documents (SBD) from the office of the head of Procurement, Federal Medical Centre, Katsina, between the hours of 9am to 4pm daily (Monday to Friday) upon payment of non-refundable tender fee of Ten Thousand Naira Only (N10, 000.00) per Lot. Payment should be made into Federal Medical Centre, Katsina’s Remita account in any reputable commercial bank in Nigeria.

5. SUBMISSION OF TENDER DOCUMENTS Prospective bidders are to submit two (2) hard copies each of the technical and financial bids with softcopy of the financial bid in MS Excel format, packaged separately in sealed envelopes and clearly marked as “Technical Bid” and “Financial Bid”. Thereafter, the two sealed envelopes should be put together in a bigger sealed envelope addressed to the Medical Director, Federal Medical Centre, Murtala Muhammed Way, P.M.B 2121, Katsina. The sealed envelope should be marked with the name of the project and Lot number. The reverse side of each sealed envelope should have the name and address of the bidder. The sealed envelope should be dropped in the designated tender box in the office of the Medical Director not later than 12:00 noon on Monday, 24 February 2020 .

6. OPENING OF TECHNICAL BIDS Only the Technical bids will be opened at 12:00 noon on Monday, 24 February 2020 in the conference hall of the Federal Medical Centre, Katsina in the presence of bidders or their representatives. Prospective bidders should ensure they sign the bid submission register in the office of the Medical Director, Federal Medical Centre, Katsina. Please note: (a) Bids must be in English Language and signed by an official authorised by the bidder; (b) Bids submitted after the deadline for submission would be returned unopened; (c) Bidders should not bid for more than two (2) lots; (d) The Federal Medical Centre, Katsina reserves the right to annul the procurement process at any time without incurring any liabilities in accordance with section 28 of the Public Procurement Act 2007.
