A. Background:

FHI 360 is implementing a Health and Nutrition Intervention for Cameroonian Refugees residing in Bence and Cross River. The projects approach to delivering quality health service delivery is by integrating services into the existing health facilities. The project supports 12 primary health facilities across Akamkpa, Bakassi. Calabar Municipal, Ikom. Obanlinku. Ogoja and Kwande LGAS The project using health systems strengthening approach, ensures supported PHCS have the capacity to meet the health needs of the Cameroonian Refugees and host community. The project is through December 2020.

FHI360 is hereby seeking is to engage pharmaceuticals distribution company (wholesale and retail) through a Master Service Agreement. The companies) through this MSA will be required to deliver upon request a monthly requirement of assorted pharmaceutical products to the project central warehouse in Ogoja Cross Rivers state. B. Evaluation Factors & Selection Criteria:
Bidders should ensure that requested information are included in the submission. Proposals will be evaluated based on the following: conformity to RFQ documentation requirement, conformity to specifications of the required drugs, competitive prices, validity of quote (quote should be valid for at least 6 months and competitive), quality and delivery terms.

C. Documentation Requirement:
Please review the table below for required components of the proposal submissions. Evaluators will either make assumptions nor draw inferences regarding missing or incomplete information.

The submission of incomplete information could result in the rejection of the proposal.

1 Company profile with registered address, functional email address and telephone number, point of contact for the entity
2 Evidence of company registration (.e. Certificate of incorporation, form CAC7 & CAC2).
3 Evidence of tax payment for the last three years
4 Provide verifiable evidence of past performance of similar projects (large volume distribution/supply), that is, signed contract/Purchase Order and evidence of completion like Job Completion Certificate and Delivery note, waybill etc for INGO, Government institution and reputable private organization
5 Three professional references, including name, phone number and organization that can attest to satisfactory performance in previous provision of related supply service.
6 Copy of company registration with Pharmacist Council of Nigeria (PCN)
7 Price Schedule for the monthly requirement of pharmaceuticals download from the above. Note, the price list/schedule should be in a separate envelope signed by the company authorized person and sealed

D. Instruction on submission:
Complete proposal including property. All documents, that is, No, 1-7 must be submitted in a scaled envelope and signed by the authorized representative of the bidding company and deliver to the below address on or before 2pm June 16th, 2020. E. Bid Clarification:
Clarification or enquiry regarding this RFQ should be sent via email Quotations Nigeria@ng.fhi360.org by 5pm June 2nd, 2020.

F. Please note:
1. The Technical Evaluation Criteria will be based on 70% and Financial Evaluation Criteria of 30%.
2. All product in your proposal must have valid NAFDAC Registration Number
3. Any proposal received by FH1360 after the deadline for submission stated herein shall be disqualified.
4. FHI360 anticipates awarding multiple contracts but reserves the right to terminate the RFQ or negotiations at any time prior to award.
5. Issuance of this RFQ in no way constitutes a commitment by FHI360 to award a contract. FHI 360 reserves the right to vary any part of this RFQ at the time of prior to award if it becomes necessary.
6. FHI360 is Value added tax (VAT) exempted, but charges Withholding Tax (WTH) in compliance with Relevant Nigerian laws.
