INVITATION TO TENDER AND EXPRESSION OF INTEREST (Eol) FOR PROJECTS UNDER THE FY2019 APPROPRIATION

1. INTRODUCTION:
University of Nigeria Teaching Hospital (UNTH) is one of the first generation Tertiary Hospital designated as Cardiothoracic Center of Excellent in Nigeria, which provides specialized medical services to the whole of South-East region and beyond. In line with BPP’s guideline and provision of the Public Procurement Act 2007, the Management of University of Nigeria Teaching Hospital Ituku- Ozalla Enugu hereby invites credible, competent and experience Bidders/Contractors and Service Providers to bid for the under listed projects.

2. PROJECT DESCRIPTION (NON-CONSULTANCY SERVICE)

Lot1S:Medical Outreach for Nnewi North/South and Ekwusigo Federal Constituency Anambra State
Lot2S:Health Checks/Outreach and Provision of Eyeglasses in Umu Ofiagu-Ukehe Enugu State
Lot3S:Medical Outreach for Otolo Nnewi, Anambra State

3. ELIGIBILITY CRITERIA
Interested and competent Service Providers/Consultants wishing to carry out any of the projects under the above three (3) categories are expected to submit the following documents in their Technical Proposals.
(i) Evidence of Business Name or Certificate of Incorporation issued by Corporate Affairs Commission (CAC) including Forms CAC 1.1 or CAC2 and CAC7;
(ii) Evidence of Company Income’s Tax Clearance Certificates (or Personal Income Tax Clearance Certificates of all the Partners) for the last three (3) years (2017, 2018 & 2019) valid till 31st December 2020;
(iii) Evidence of current Pension Commission (PENCOM) Compliance Clearance Certificate valid till 31st December 2020;
(iv) Evidence of current Industrial Training Fund (ITF) Compliance Certificate valid till 31st December 2020;
(v) Evidence of current Nigeria Social Insurance Trust Fund (NSITF) Compliance Certificate valid till 31st December 2020;
(vi) Evidence of current registration on the National Database of Contractors, Consultants and Service Providers by submission of Interim Registration Report (IRR) expiring on 30 June 2020 or valid certificate issued by Bureau of Public Procurement (BPP);
(vii) A sworn affidavit disclosing whether or not any officer of the relevant committees of University of Nigeria Teaching Hospital (UNTH) or the Bureau of Public Procurement is a former or present Director, shareholder or has any pecuniary interest in the bidder and to confirm that all information presented in its bid are true and correct in all particulars;
(viii) Company’s Audited Account for the last three (3) years (2017, 2018 & 2019);
(ix) Evidence of Financial Capability to execute the project by submission of reference letter from a reputable commercial bank indicating willingness to provide credit facility for the execution of the project when needed;
(x) Evidence of current licenses for all the medical personnel involved e.g. Doctors, Pharmacist, Nurses, Laboratory Scientist etc. must be attached;
(xi) Verifiable documentary evidence of experience in at least three (3) jobs of similar nature and complexity, executed in the last five (5) years including Letters of Award and Job Completion Certificate.
(xii) For Joint Venture/Partnership, Memorandum of Understanding (MoU) should be provided (CAC, Tax Clearance Certificate, Pension Compliance Certificate, ITF Compliance Certificate, NSITF Compliance Certificate, IRR& Sworn Affidavit are compulsory for each JV partner);
(xiii) All documents for submission must be transmitted with a Covering/Forwarding letter under the Company/Firm’s Letter Head Paper bearing among other the Registration Number (RC) as issued by the Corporate Affairs Commission (CAC), Contact Address, Telephone Number (preferably GSM No) and E-mail address. The Letterhead Paper must bear the names and Nationalities of the Directors of the Company at the bottom of the page, duly signed by the authorized officer of the firm.

4. SUBMISSION OF TENDER DOCUMENTS:
Interested Firms are to submit three (3) bound copies of Expression of Interest (Eol) documents in a sealed envelope and clearly marked “Expression of Interest (Eol)”, with the Project Name and Lot number clearly written on the top right-hand corner, addressed and returned to:

Furthermore, the reverse of each sealed envelope should have the name and address of the bidder and dropped in the designated Tender box at the office of Head, Procurement Department between 8am to 4pm daily not later than 12:00 noon on Monday 25 May, 2020.

5. OPENING OF EOI DOCUMENTS:
Expression of Interest (Eol) documents will be opened immediately after the deadline for submission at 12:00 noon on Monday 25 May, 2020 at the UNTH Board Room, Ituku-Ozalla, Enugu, in the presence of bidders or their representatives, Civil Society Organisations and Professional Bodies.
Only successful firms (Consultants) shall be invited at a later date for collection of Request for Proposal (RfP).

6. GENERAL INFORMATION
(a) EOls must be in English Language and signed by an official authorized by the bidder;
(b) EOls submitted after the deadline for submission would be returned un-opened;
(c) Bidders should not bid for more than two (2) Lots;
(d) All cost will be borne by the bidders;
(e) The University of Nigeria Teaching Hospital is not bound to pre-qualify any bidder and reserves the right to annul the Procurement process at any time without incurring any liabilities in accordance with Section 28 of the Public Procurement Act 2007.
(f) Any unsigned document will be disqualified.
(g) All Bidding Documents submitted will be subjected to Due Diligence (registration with CAC, tax clearance certificates, etc.) checks in accordance with the Public Procurement Act (P PA) 2007.
(h) The final conclusion of the Procurement Process for all Projects listed above is subject to Budgetary Releases from the Treasury under the FY 2018Appropriation
